摘要
目的探讨急性髓系白血病患者巨噬细胞炎症蛋白-1α(MIP-1α)的表达与预后的相关性。方法采用回顾性研究,2012年2月至2016年8月选择在本院诊治的急性髓系白血病患者96例作为观察组,同期选择在本院进行体检的成人健康志愿者作为对照组,两组均进行IP-1α的表达检测,调查观察组患者的临床特征与预后生存情况。结果观察组与对照组的MIP-1αmRNA相对表达量分别为(2.94±0.55)和(0.78±0.64),观察组明显高于对照组(P<0.05)。在观察组中,MIP-1αmRNA相对表达量在不同免疫分型、核型中的表达差异明显(P<0.05),在性别、年龄、体质量指数中的表达无明显差异(P>0.05)。观察组96例患者随访至今,生存72例,生存率为75.0%。生存患者的MIP-1αmRNA相对表达量为(1.87±0.52),明显低于死亡患者的(4.83±0.72)(t=7.398,P<0.05)。多因素Cox回归模型分析显示免疫分型、核型、MIP-1αmRNA都为影响急性髓系白血病患者预后生存的独立危险因素(P<0.05)。结论急性髓系白血病患者MIP-1α呈现高表达状况,与患者的免疫表型、核型、预后生存呈相关性,且是影响患者预后生存的主要独立危险因素。
Objective To investigate the relationship between the expression of macrophage inflammatory protein-1α(MIP-1α)and prognosis in patients with acute myeloid leukemia(AML).Methods Used a retrospective study,96 patients with acute myeloid leukemia were treated in our hospital from February 2012 to August 2016 were selected as the observation group,At the same time,96 cases of the adult healthy volunteers in our hospital were selected as the control group.Two groups were tested for the expression of MIP-1α.The clinical features and prognosis of the patients in the observation group were investigated.Results The relative expression of MIP-1αmRNA in the observation group and the control group were 2.94±0.55 and 0.78±0.64 that the observation group was significantly higher than that of the control group(P〈0.05).In the observation group,the relative expression of MIP-1 mRNA were significantly different in different immune type and karyotype(P〈0.05),and there were no significant difference in sex,age,body mass index(P〈0.05).Patients in the observation group were followed up to now,72 cases were survival,the survival rate was 75%.The relative expression level of MIP-1αmRNA in survival patients was 1.87±0.52,which was significantly lower than that of the death patients of 4.83±0.72(t=7.398,P〈0.05).Multivariate Cox regression analysis showed that the immunophenotype,karyotype and MIP-1αmRNA were independent risk factors for the prognosis of patients with acute myeloid leukemia(P〈0.05).Conclusion The expression of MIP-1αin patients with acute myeloid leukemia is highly,it is correlated with the immunophenotype,karyotype and prognosis of patients,and it is the main independent risk factor for influencing the prognosis of patients.
