肺部肿块CT引导下经皮肺穿刺活检的临床诊断价值分析

The Clinical Value of CT-guided Percutaneous Needle Biopsy in Diagnosis of Lung Tumor

目的:探讨CT引导下经皮肺穿刺活检在肺部肿块临床诊断中的应用价值。方法:本次研究选取了2010年1月-2017年3月笔者所在医院行CT引导下经皮肺穿刺活检及外科手术治疗的肺部肿块患者109例,并将经皮肺穿刺活检结果与患者的大体标本病理学诊断结果相比分析。结果:109例患者均穿刺成功,穿刺成功率为100%。穿刺活检结果,阳性99例,阴性10例。109例患者经大体标本病理学诊断证实,恶性病变84例,占比77.1%;良性病变16例,占比14.7%;阴性9例,占比8.2%。与大体标本病理学诊断结果相比,CT引导下经皮肺穿刺活检的灵敏度为97%(97/100),特异度为78%(7/9),诊断准确率为95%(104/109)。结论:CT引导下经皮肺穿刺活检在肺部肿块的诊断中具有较高的敏感性、特异性和准确率,且具有创伤较小、安全性较高、成本较低等临床优势,可以在肺部肿块的临床诊断中进行推广应用。

Objective： To evaluate the clinical value of CT-guided percutaneons needle biopsy in the diagnosis of pulmonary lumps.Method： I09 patients with lung tumor underwent CT-gnided percutaneous needle biopsy and surgical treatment between January 2010 and March 2017 were selected, the biopsy and pathological diagnoses were compared.Result： All 109 patients had succeed in biopsy, with 99 positive cases and 10 negative cases.Compared with the pathological diagnoses after surgery which showed 84 cases of malignant tumor, 16 cases of benign tumor and 9 negative cases, the sensitivity of CT-guided percutaneous needle biopsy was 97%（97/100）, while the specificity was 78%（7/9）, and the accurate rate was 95%（104/109）.Conclusion： CT-guided percutaneous needle biopsy in the diagnosis of pulmonary lumps has relatively high sensitivity, specificity and accurate rate.With the advantages of less trauma, high safety and low cost as well, it can be widely applied in the clinical diagnosis of lung tumor.
