根管治疗结合牙周翻瓣术对牙髓-牙周综合征患者牙周相关指数及细菌感染与清除情况的影响

Effect of root canal therapy combined with periodontal flap surgery on periodontal related indexes,bacterial infection and clearance in patients with pulp-periodontal syndrome

目的探讨根管治疗结合牙周翻瓣术对牙髓-牙周综合征的临床疗效。方法随机数字法将101例牙髓-牙周综合征患者分为观察组与对照组,观察组行根管治疗结合牙周翻瓣术,对照组则给予单纯根管治疗,比较2组临床疗效、治疗前后牙周相关指数及细菌学指标变化情况。结果观察组治疗总有效率94.12%显著高于对照组的76.00%(P<0.05);与治疗前比较,2组治疗后菌斑指数、牙周袋探诊深度、牙龈指数、龈沟出血指数均显著下降(P<0.05),且观察组治疗后上述指标显著低于对照组(P<0.05);2组治疗后细菌感染率较治疗前显著下降(P<0.05),2组治疗后细菌感染率、细菌清除率比较差异无统计学意义(P>0.05)。结论根管治疗结合牙周翻瓣术治疗牙髓-牙周综合征疗效较好,能明显改善牙周健康状态,同时能有效减少细菌感染发生。

Objective To investigate the clinical efficacy of root canal therapy combined with periodontal flap surgery in the treatment of pulp-periodontal syndrome. Methods 101 patients with pulp-periodontal syndrome were randomly divided into the observation group and the control group. The observation group was treated with root canal therapy and periodontal flap surgery, while the control group was treated with root canal therapy alone. The clinical efficacy, changes of periodontium related indexes and bacteriological indexes before and after treatment were compared between the two groups. Results The total effective rate of treatment in the observation group （94.12%） was higher than that in the control group （76.00%） （P〈0.05）. After treatment, the plaque index, probing depth, gingival index and sulcus bleeding index were significantly lower （P〈0.05）, and the above indexes in the observation group after treatment were significantly lower than those in the control group （P〈0.05）. After treatment, the bacterial infection rates in the two groups were significantly lower （P〈0.05） but there were no significant differences in the bacterial infection rate or bacterial clearance rate between the two groups （P〉0.05）. Conclusion Root canal therapy combined with periodontal flap surgery is effective in the treatment of pulp-periodontal syndrome. The regimen can significantly improve periodontal health, and reduce the incidence of bacterial infection.
