深圳市龙岗区人感染H7N9禽流感流行特征与活禽市场休市效果评价 被引量:9

Epidemic characteristics of human H7N9 avian influenza infection and evaluation of live poultry market in Longgang District of Shenzhen

摘要 目的通过分析辖区内人群感染H7N9禽流感的疫情特征以及活禽市场休市前后的疫情变化,以期为预测H7N9禽流感病毒的流行趋势,探讨活禽市场管理体系的重构提供理论依据。方法收集全区各医疗机构报告的确诊人感染H7N9病例资料进行流行病学调查分析;开展多轮次活禽市场外环境监测,收集不同活禽市场管理措施的信息资料以及活禽市场环境标本进行流感病毒核酸检测;全区各个活禽交易市场均根据广东省规定的"1110"措施进行常规市场管理,每月统一休市1d,于2015年1月31日起全市开始为期10日的全市活禽市场休市;比较分析两种休市措施休市前后疫情变化情况。结果环境标本流感病毒核酸检测结果显示有138份标本为A型流感病毒核酸阳性,H5、H9、H7亚型流感病毒核酸分别检出24、37、3份,3份H7亚型流感病毒核酸检出的标本中有1份H7N9;确诊病例中不存在性别差异(14/11),年龄大于40岁的中老年患者比例较高(17/25),有活禽接触史及活禽市场接触史的病例比例较高(23/25),春冬二季多发(23/25);休市10d后龙岗区活禽市场外环境中A型流感、H9亚型流感病毒核酸检出率明显较"1110"措施休市1d检出率低,H5亚型流感病毒核酸检出率无显著差异,两种措施后H7亚型流感病毒核酸检出率均为0。结论了解辖区内H7N9禽流感疫情的流行特征以及不同活禽市场管理措施的控制效果,对预测H7N9禽流感病毒流行趋势、重构活禽市场管理体系具有重要的指导意义,如何进行活禽市场休市仍需要进一步的研究。 Objective To analyze the epidemic characteristics of HTN9 avian influenza infection in the area and the epidemic changes before and after the live pouhry market closed, in order to provide a theoretical basis for the prediction of the epidemic trend of I-ITN9 avian influenza virus and the reconstruction of the management system of live poultry market. Methods The data of the confirmed cases of human infection H7N9 reported by medical institutions in the region were selected. Multiple rounds of live poultry market environmental monitoring was carried. Different live poultry market management measures of information and live poultry market environmental samples for detection of influenza virus nucleic acid were collected. The region' s live poultry trading market were in accordance with the provisions of the province of Guangdong "1110" measures for conventional market management, the monthly unification of 1D, and since January 31, 2015 the city began a period of 10 days of the city' s live poultry market closed. The epidemic situation of two kinds of measures before and after the outbreak was comparative analyzed. Results The detection results were influenza virus nucleic acid environment showed that 138 specimens for influenza virus nucleic acid positive, the HS, H9, H7 subtype influenza virus nucleic acid were detected 24,37,3 copies, 3 specimens of H7 subtype influenza virus nucleic acid detected in the sample of 1 H7N9. There was no gender difference (14/11) in the confirmed cases, and the proportion of elderly patients over 40 years old was higher(17/25), and there was a higher proportion of cases of live poultry exposure and live poultry market contact history(23/25), multiple in spring and winter(23-25). The detection rate of Avian. H9 subtypeinfluenza live in the external environment of the Longgang market closed hugh 10D was significantly lower than the "1110" measures closedlD, there was no significant difference in the detection rate of H5 subtype influenza virus nucleic acid. The detection rate of H7 subtype influenza virus was 0 after the two measures. Conclusion Understanding the epidemic characteristics of H7N9 avian influenza in the area and the control effect of different live poultry market management measures is a great significance to predict the epidemic trend of HTN9 avian influenza virus and reconstruct the management system of live poultry market. We still needs further research about how to live poultry market.
