摘要
目的探讨经皮椎体后凸成形术(PKP)治疗老年骨质疏松性压缩性骨折的临床效果。方法选取我院于2013年12月~2015年3月骨科收治的老年骨质疏松性压缩性骨折患者56例,局部麻醉下行PKP治疗,记录并比较手术前后及末次随访时,患者的伤椎椎体高度、后凸角度、视觉模拟评分法(VAS)和Oswestry指标评分,比较术后1周及末次随访时,患者的并发症情况。结果 56例患者术后1周和末次随访的伤椎椎体高度、后凸角度、VAS和Oswestry各项临床指标评测值均优于术前,差异具有统计学意义(P<0.05)。患者末次随访的并发症发生率为26.79%,明显高于术后1周的3.57%,差异具有统计学意义(P<0.05)。结论作为治疗老年骨质疏松性压缩性骨折的一项微创外科手术,PKP具有操作简便、止痛效果佳、疗效好的优点,能有效矫正后凸畸形并恢复椎体高度,缓解患者的临床症状,其短期治疗效果值得肯定。
Objective To explore clinical effect of penetrating keratoplasty(PKP) in treatment of senile osteoporotic compression fractures. Methods 56 patients with senile osteoporotic compression fractures who were admitted to department of orthopedics in our hospital from December 2013 to March 2015 were selected. They were given PKP under local anesthesia. The height of injured vertebrae, kyphosis angle, visual analogue scale(VAS) and Oswestry index score of patients in 1 week after operation and final follow-up were recorded and compared. Complications of 1 week after operation and final follow-up were compared. Results Evaluation values such as the height of injured vertebrae, kyphosis angle, VAS and Oswestry index of 56 patients in 1 week after operation and final follow-up were all superior to those before operation(P 〈 0.05), and the differences were statistically significant. Complication rate of patients in final follow-up was 26.97%, significantly higher than that of 1 week after operation 3.57%. The difference was statistically significant(P 〈 0.05). Conclusion As a miniature injury surgery in treatment of senile osteoporotic compression fractures, PKP has advantages such as convenient operation, good antalgic effect and good clinical effect. It can effectively correct kyphosis deformity, restore height of vertebrae and relieve clinical symptoms of patients. In addition, its short-term curative effect is worthy of approval.
