EdinburghⅡ型A组2型锁骨骨折四种不同手术内固定方式的临床对比研究

Clinical comparative study of four different methods of internal fixation for EdinburghⅡgroup A type 2 clavicle fracture

目的比较四种固定方式在EdinburghⅡ型A组2型锁骨骨折治疗中的效果,阐明其临床应用价值。方法选择我院接收的EdinburghⅡ型A组2型锁骨骨折患者120例,随机分为A、B、C、D四组,分别实施克氏针内固定、空心加压螺钉内固定、重建钢板内固定和交锁髓内钉固定治疗,就并发症发生情况、疼痛评分或是肩功能评价、手术的一般情况等对四组患者进行对比。结果 D组的效果显著优于其他三组,与其余三组进行统计学比较,差异有统计学意义(P<0.05);而其余三组进行两两比较,差异无统计学意义(P>0.05)。结论相较于其他三种固定方式,交锁髓内钉固定治疗效果更为显著,能够降低患者并发症的发生几率,减轻患者的疼痛感,能够有效缩短手术时间,减少手术中出血量,促进患者病情和肩功能的快速恢复,有益于提高患者的生活质量;临床应用价值高,值得广泛推行。

Objective To compare the effect of four different methods of internal fixation for Edinburgh Ⅱ group A type 2 clavicle fracture,and to clarify its clinical application value. Methods 120 cases of patients with Edinburgh Ⅱ group A type 2 clavicle fracture cured in our hospital were selected and randomly divided into group A,group B,group C and group D.They were treated with kirschner wire fixation,cannulated compression screw internal fixation,reconstruction plate internal fixation and interlocking intramedullary nail fixation,respectively. The incidence of complications,pain score or shoulder function evaluation,the general situation of the operation of the four groups were compared. Results The effect of group d was significantly better than the other three groups,compared with the other three groups,（P 〈 0.05）,and it was statistically significant.While the other three groups were compared with each other, （P 〈 0.05）,it was not statistically different. Conclusion Compared with the other three fixation methods,the effect of interlocking intramedullary nail fixation is more significant.It can reduce the incidence of complications,reduce the pain of patients,effectively shorten the operation time,reduce the amount of bleeding in the operation,and promote the rapid recovery of the patient＇ s condition and shoulder function.h is helpful to improve the quality of life of patients.It has high clinical application value.h is worthy of extensive implementation.
