山楂消脂胶囊对肥胖症患者炎性状态及脂多糖水平的影响

Effects of Shanzha xiaozhi capsule on inflammation and lipopolysaccharide level in obese patients

目的研究山楂消脂胶囊对肥胖症患者体重、机体脂肪含量、胰岛素抵抗、炎性状态及脂多糖水平的影响。方法将108例肥胖症患者按随机数字法分成对照组（46例）和药物组（62例）。对对照组进行饮食和运动的生活方式指导。对药物组在生活方式指导的基础上，予山楂消脂胶囊口服，一次0．7g，一日3次。疗程为12周。分别在治疗前、后测量两组的体重，运用双能X线吸收测量法测量机体脂肪含量和内脏脂肪量，采用化学发光法检测空腹胰岛素，采用放射免疫法检测血清肿瘤坏死因子-α（TNF-α）、白细胞介素（IL）-18和IL-6，采用ELISA法检测血清脂多糖水平。结果两组治疗后的体重及体重指数、全身脂肪含量和内脏脂肪量、空腹胰岛素水平和稳态模型评估-胰岛素抵抗指数（HOMA—IR）、TNF-α、IL-1p、IL-6、脂多糖水平均显著下降（t=2．148～12．131，P均〈0．05），药物组比对照组下降更显著（Z=-3．843～-2．875，P均〈0．01）。结论山楂消脂胶囊可降低肥胖症患者体重，减轻体内炎性状态。后者可能与山楂消脂胶囊减少内脏脂肪和降低血清脂多糖水平有关。

Objective To investigate the effects of Shanzha xiaozhi capsule on weight, body fat percentage, insulin resistance, inflammation and lipopolysaccharide（LPS） level in obese patients. Methods A total of 108 obese patients were enrolled and divided into control group （ n = 46） and drug group （ n = 62） according to the random number method. Patients in control group received a conventional intervention of diet and exercise. Patients in drug group were prescribed with Shanzha xiaozhi capsule （0.7 g a time, 3 times a day） on the basis of the conventional diet and exercise intervention for 12 weeks. Before and ＇after the in- tervention, weights were measured in two groups. The body fat percentage and visceral fat mass were measured by Dual-energy X-ray. Fasting plasma insulin was detected by chemiluminescence. Serum tumor necrosis factor-or （TNF-ct） , interleukin-1β （ IL-1β ） and interleukin-6 （ IL-6 ） were detected by radioimmunoassay. Serum LPS was detected by ELISA kit. Results The weight and body mass index, body fat percentage and visceral fat mass, fasting plasma insulin and the homeostasis model of assessment for insulin resistance index （HOMA-IR） ,TNF-α, IL-1β, IL-6 and LPS were decreased significantly in both groups after treatment （t=2.148-12.131, all P〈0.05）, but decreased greater in the drug group（Z= -3.843- -2.875, all P〈0.01）. Conclusion In addition to its contribution to weight loss in obese patients, Shanzha xiaozhi capsule also contributes to the reduction of inflammation, which may be related to the reduction of visceral fat mass and serum LPS.
