摘要
目的分析围手术期运用肠内营养与肠外营养两种支持治疗方式对胃癌根治术患者术后营养状况、术后并发症等方面的影响。方法选取笔者所在医院收治的48例胃癌根治术患者,采用随机数字表法将其分为肠内营养组(EN组)和肠外营养组(PN组),各24例,分别在术后第1天开始给予肠内营养液及肠外营养液进行营养支持治疗,在术前1天及术后第8天检测血清白蛋白、血清前白蛋白,并比较两组术后并发症发生情况。结果两组患者血清白蛋白和血清前白蛋白较术前均升高,EN组较PN组升高更为显著,两组比较差异有统计学意义(P<0.05)。EN组并发症发生率低于PN组,差异有统计学意义(P<0.05)。结论胃癌根治术围手术期应用肠内营养比肠外营养具有优势,有利于降低术后并发症发生率,缩短住院时间,增强患者的体质。
Objective To analyze the influence of two kinds of support methods of enteral nutrition and parenteral nutrition on nutritional status and postoperative complications in patients undergoing radical gastrectomy for gastric cancer. Methods In our hospital, 48 cases of patients with gastric cancer were randomly divided into enteral nutrition group(group EN) and parenteral nutrition group(group PN), 24 cases in each group, respectively on the first postoperative day was given enteral nutrition and parenteral nutrition for support treatment. Serum albumin and serum prealbumin were detected at one day before operation and eighth days after operation, and the postoperative complications of the two groups were compared. Results The serum albumin and serum prealbumin in the two groups were higher than those before operation, and the EN group was more significantly higher than the group PN, the difference between the two groups was statistically significant(P〈0.05). The incidence of complications in group EN was lower than that in group PN, the difference was statistically significant(P〈0.05). Conclusion The application of enteral nutrition in the perioperative period of radical gastrectomy for gastric cancer has more significant advantages than parenteral nutrition, which is helpful to reduce the incidence of postoperative complications, shorten the length of stay and enhance the patient's constitution.
