“汉堡式”敷料在造口周围粪水性皮炎护理中的应用

Application of “Hamburg” Dressing in Nursing of Fecal Water Dermatitis Around the Ileal Stoma

目的观察回肠造口周围粪水性皮炎患者使用"汉堡式"敷料联合导尿管治疗的效果。方法将64例回肠造口周围粪水性皮炎患者随机分成两组,对照组32例,更换造口底盘时使用20Fr气囊导尿管引流封堵造口,喷洒造口粉及不含酒精保护膜1次,贴上水胶体敷料,两件式造口袋粘贴造口。实验组32例,方法是在对照组的基础上使用自制"汉堡式"敷料,比较两组患者7 d内该部位皮肤愈合、疼痛程度、费用情况。结果使用自制"汉堡式"敷料可使造口周围粪水性皮炎愈合的时间快,同时,可减少患者的疼痛,更换造口的频率有所下降,差异有统计学意义,P<0.05。结论 "汉堡式"敷料治疗回肠造口粪水性皮炎效果好,不但能够在短时间内促进造口周围皮肤愈合,减轻患者疼痛,同时增加底盘的牢固性,减少频繁更换造口底盘给患者带来的不适感,减少底盘消耗,减轻患者经济负担。

Objective To observe the effect of ＂Hamburg＂ dressing combined with catheter in fecal water dermatitis around the ileal stoma. Methods 64 patients with fecal dermatitis around ileal stoma were randomly divided into two groups. 32 patients in the control group were inserted 20 Fr balloon catheter to closure and drainage stoma, then sprayed stoma powder and alcohol free protective film once, pasted the water colloid dressing, at last two-piece pockets passted stoma, when replacingthe stoma chassis. 32 patients in the control group, the method was based on the control group using homemade hamburger dressing. The skin healing, pain degree and expenditure of 7d in two groups were compared. Results The use of homemade ＂Hamburger＂ dressings can heal dung dermatitis acceleratedly which around stoma,reduce the pain of the patient and the frequency of replacement stoma.There was significant difference between the two groups in cure rate, P〈0.05. Conclusion ＂Hamburger＂dressing is effective in treating fecal water dermatitis around the ileal stoma, not only to promote the healing of the skin around the stoma, reducing pain in patients, but also to increase the firmness of the chassis, extending the bonding time, reducing the discomfort accompany with frequent replacement of chassis, chassis consumption and the financial burden of patients. It is worthy of clinical application promotion.
