拔牙窝冲洗后牙槽窝对患者血液炎性因子IL-1、IL-6、TNF-α浓度的影响情况分析

Effects of rinsing tooth socket after tooth extraction on IL-1 beta,IL-6 and TNF in tooth socket liquid and in serum of patients with chronic periodontitis

目的探讨拔牙后对牙槽窝进行冲洗对牙槽窝液、血清中白介素-1β(IL-1β)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平的影响。方法选取口腔科行牙拔除术的90例患者,采用抛掷硬币的方法分为搔刮组(对拔牙窝进行搔刮处理)、冲洗组(搔刮+1.0%氧化氢溶液+0.9%氯化钠溶液冲洗),每组45例,采用酶联免疫吸附法(ELISA)法检测并对比牙槽窝液、血清中IL-1β、IL-6、TNF-α的水平。结果搔刮组患者的血清中IL-1β、IL-6、TNF-α水平均高于冲洗组,差异有统计学意义(P<0.05);搔刮组患者的牙槽沟液中IL-1β、IL-6、TNF-α的水平均高于冲洗组,差异有统计学意义(P<0.05);拔牙患者血清与牙槽沟液中IL-1β、IL-6、TNF-α的水平均呈显著的正相关(P<0.05)。结论牙周慢性炎症拔牙患者拔牙后对牙槽窝进行冲洗有利于减轻炎性反应,降低血清及牙槽沟液中的炎性因子水平。

Objective To investigate the effects of rinsing tooth socket after tooth extraction on the levels of interleukin-1 beta（IL-1β）,interleukin-6（IL-6） and tumor necrosis factor alpha（TNF） in tooth socket liquid and in serum of patients with chronic periodontitis. Methods Ninety patients who underwent tooth extraction in our hospital from January2014 to December 2015 were divided two groups by the method of throwing a coin： scoraping group（n = 45） and rinsing group（n = 45）. The patients in scoraping group were treated by scoraping tooth crypt after tooth extraction,however,the patients in rinsing group were treated by scoraping tooth crypt plus rinsing tooth crypt with 1. 0% hydrogen peroxide solution ＋ 0. 9%sodium chloride solution,then the levels of IL-1β,IL-6,TNF-α in tooth socket liquid and in serum of patients were detected by ELISA. Results The levels of IL-1β,IL-6,TNF-α in serum of patients in scoraping group were significantly higher than those in rinsing group（P〈0.05）. Moreover the levels of IL-1β,IL-6,TNF-α in tooth socket liquid in scoraping group were significantly higher than those in rinsing group（P〈0.05）. The levels of IL-1β,IL-6,TNF-α in serum of patients after tooth extraction were positively correlated to those in tooth socket liquid（P〈0.05）. Conclusion To rinse tooth socket after tooth extraction in patients with chronic periodontitis is beneficial to relieve inflammatory reaction and to reduce the levels of inflammatory factors in serum and in dental alveoli liquids.
