慢性肾衰血液透析患者透析性低血压的影响因素

Analysis on relevant factors of chronic renal failure patients with hemodialysis hypotension

目的研究分析慢性肾衰维持性血液透析患者发生透析性低血压的相关因素。方法回顾性分析2014年2月至2016年2月在郑州颐和医院行维持性血液透析的慢性肾衰患者42例,收集患者的年龄、原发疾病、血红蛋白、超滤量、甲状旁腺激素。分析这些临床资料与患者发生低血压症之间的关系。结果高龄患者(>60岁)、糖尿病肾病患者、超滤量高(>2.5kg)、继发性甲旁亢(>500Pg/ml)患者低血压发生率显著增加,有统计学意义(P<0.05);多因素Logistic回归分析显示:年龄>60岁、超滤量>2.5kg、糖尿病肾病是低血压发生的主要危险因素。结论高龄、患有糖尿病肾病以及超滤量高是慢性肾衰维持性血液透析患者低血压发病的主要危险因素。

Objective To analysis the relevant factors of chronic renal failure patients with hemodialysis hypotension.Methods Totally 42 chronic renal failure patients who underwent maintenance hemodialysis（ MHD） from Feb. 2014 to Feb. 2016 were analysed retrospectively. Age,primary diseases,hemoglobin,ultrafiltration volume（ UFV）,parathyroid hormone（ PTH）,were recorded and analysis. Results The incidence of hypotension was significantly higher in elderly patients（ age 〉60 years）,diabetic nephropathy（ DN） patients,UFV 〉2. 5kg and secondary hyperparathyroidism（ PTH〉 500 pg/ml）,with statistical significance. Multivariate regression analysis showed elderly（ 60 years）,with DN,and higher UFV（ 〉2. 5kg） were associated with hemodialysis hypotension. Conclusion Elderly,DN,higher UFV are the high risk factors for hemodialysis hypotension among MHD patients.
