泪道冲洗联合泪道探通术治疗新生儿泪囊炎50例体会

Clinical experience of initial lacrimal passage irrigation and probing in 50 cases of neonatal dacryocystitis

目的分析探讨泪道冲洗联合泪道探通术在新生儿泪囊炎中的疗效。方法临床病例系列研究。对2015年9月至2016年12月在中国医科大学附属第一医院眼科，常规泪囊按摩结合局部点用抗生素滴眼液无效的50例58只眼新生儿泪囊炎患儿，在局部麻醉下行泪道冲洗联合泪道探通术，观察治疗效果。结果本研究50例58只眼中，患儿2~6月龄19只眼，行一次泪道冲洗联合泪道探通术治愈率100%（19/19），6~18月龄39只眼，行一次泪道冲洗联合泪道探通术治愈28只眼，二次探通治愈7只眼，未治愈4只眼。结论对于保守治疗无效的新生儿泪囊炎患儿，月龄〉2月者应尽早行泪道冲洗联合泪道探通术，安全有效且可行。

ObjectiveTo evaluate the therapeutic effectiveness of initial lacrimal passage irrigation and probing in neonatal dacryocystitis.MethodsFifty cases （58 eyes） with neonatal dacryocystitis which had no effect by routine dacryocyst massage with local antibiotics ophthalmic solutions were treated with passage irrigation and probing under local anesthesia to observe the therapeutic effectiveness.ResultsIn this study, the success rate of initial lacrimal passage irrigation and probing for 50 cases （58 eyes） at 2-6 months of age was 100% （19/19）, 39 eyes were in aged 6-18 months, in which 28 eyes cured with once probing, 7 eyes cured with twice probing, 4 eyes failed.ConclusionsIn neonatal dacryocystitis, aged above 2 months, lacrimal passage irrigation and probing should be performed as soon as possible which is also safe and feasible.
