血清TK1联合CA19-9在胰腺癌和胰腺炎鉴别诊断中的应用价值研究

Value of serum TK1 combined with CA19-9 in differential diagnosis of pancreatic cancer and pancreatitis

目的:探讨联合检测胰腺癌、胰腺炎患者的血清胸苷激酶(TK1)、糖类抗原19-9(CA19-9)鉴别诊断胰腺癌、胰腺炎的临床价值。方法:选取2015年6月—2016年6月在本院接受诊治的87例胰腺癌患者、急性胰腺炎患者60例进行分析,检测两组患者的血清TK-1、CA19-9水平,计算二者联合检测时鉴别诊断胰腺癌、胰腺炎的临床价值。结果:胰腺癌组患者的血清TK-1、CA19-9水平均高于胰腺炎组,差异具有统计学意义(P<0.05);胰腺癌组患者的血清ALT、AST、ALP水平均低于胰腺炎组,差异具有统计学意义(P<0.05);血清TK-1+CA19-9鉴别诊断胰腺癌、胰腺炎的灵敏度为90.80%、特异度95.00%、漏诊率9.20%、误诊率5.00%,ROC曲线下面积AUC值为0.926。结论:联合检测胰腺癌、胰腺炎患者的血清TK1、CA19-9鉴别诊断胰腺癌、胰腺炎具有较高的临床价值。

Objective： To investigate the clinical value of combined detection of serum thymidine kinase（ TK1）and carbohydrate antigen 19-9（ CA19-9） in the differential diagnosis of pancreatic cancer and pancreatitis in patients with pancreatic cancer and pancreatitis. Methods： Eighty seven cases of patients with pancreatic cancer and 60 cases of acute pancreatitis patients from June 2015 to June 2016 were selected and analyzed in our hospital.Serum TK-1,CA19-9 level were detected,and the combined detecting value of differential diagnosis of pancreatic cancer and pancreatitis was evaluated. Results： Serum TK-1,CA19-9 in pancreatic cancer patients was higher than the average in pancreatitis group,the difference had statistical significance（ P〈0. 05）; serum ALT,AST and ALP levels in pancreatic cancer patients were lower than those in the acute pancreatitis group,the difference being statistical significant（ P〈0. 05）; the sensitivity of serum TK-1 ＋ CA19-9 in differential diagnosis of pancreatic cancer and pancreatitis was 90. 80%,specificity was 95. 00%,misdiagnosis rate 9. 20%,misdiagnosis rate 5. 00%,area under the ROC curve AUC 0. 926. Conclusion： Combined detection of serum TK1 and CA19-9in patients with pancreatic cancer and pancreatitis has high clinical value in the differential diagnosis of pancreatic cancer and pancreatitis.
