The Choice of Lower Extremity Deep Frostbite Amputation Plane

目的探讨对下肢深度冻伤截肢平面的选择和对于治疗的效果进行分析。方法选择我院2007年1月—2016年12月收治的下肢严重冻伤的患者28例作为研究对象,本组病例均为下肢深度冻伤,受伤部位可见水疱、皮肤呈青紫色、灰白或紫黑色。针对患者具体的病情,对患者提出了截肢方法和残端处理建议,通过采用不同的截肢术式,观察患者的治疗效果。结果本组患者手术后,残端切口大部愈合良好,植皮及转移皮瓣均获成活,无骨髓炎、败血症,DIC等并发症发生,一期愈合率为92%,患者均康复出院,为后期装配和运用假肢提供了保障。结论对于出现了下肢深度冻伤的患者进行截肢方法和残端处理,可以有效地促进深度冻伤患者残端的愈合,截肢后便于安装和使用假肢,提高患者的生活质量。

Objective To investigate the choice of the lower limb deep frostbite limb plane and the effect of the treatment. Methods A total of 28 patients with severe frostbite of lower extremities were treated in our hospital from January 2007 to December 2016. The injured area was seen with blisters, the skin was appears with bluish purple, gray or purple black. In view of the patient＇s specific condition, the amputation method and stump treatment suggestion were put forward. The therapeutic effect of the patients was observed. Results Most of the incisions healed well after the operation, and the skin grafts and the flaps were survived without complications such as osteomyelitis, no septicemia, DIC cases happens, healing rate was 92%, patients were discharged, after the period for easy assembly and provides security by prosthesis. Conclusion For patients with deep frostbite of the lower limb amputation method and the end of treatment, can effectively avoid the complications of patients after frostbite, After amputation, it is easy to install and use artificial limbs, which could improve patients＇ quality of life.
