羟氯喹对系统性红斑狼疮患者孕期血脂、同型半胱氨酸及心功能的影响

Effect of blood lipid,homocysteine and cardiac function with hydroxychloroquine on systemic lupus erythematosus patients during pregnancy

(1)目的探讨羟氯喹对系统性红斑狼疮(SLE)患者孕期血脂、同型半胱氨酸(Hcy)及心功能的影响。(2)方法 61例SLE妊娠患者随机分成两组,其中观察组31例为羟氯喹加激素治疗,对照组30例采用单纯激素治疗,分析两组患者孕期血脂、Hcy及心功能的改变。(3)结果观察组总胆固醇(TC)、甘油三脂(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)和Hcy均低于对照组,两组比较差异有统计学意义;血清Hcy、TC、TG、LDL-C与SLE疾病活动度(SLEDAI)评分呈正相关,血清Hcy水平与TC、TG、LDL-C呈正相关;心功能两组比较则无明显改变。(4)结论 SLE患者孕期使用羟氯喹可明显改善脂代谢,而对心功能则影响不大。因此认为SLE患者孕期使用羟氯喹是有益的,可有效改善糖脂代谢,降低SLEDAI评分和心脑血管疾病的发病风险。

Objective To explore the effect of blood lipid, homocysteine with hydroxychloroquine on systemic lupus erythematosus patients during pregn and cardiac functlon ancy.Methods A total of 61 pregnancy cases with SLE were randomly divided into two groups.The 31 cases observation group with hydroxychioroquine plus hormone therapy and 30 cases control group with hormone therapy.The blood lipid, homocysteine and cardiac function change were analyzed in the two groups.Results The observation group total cholesterol（TC）, three glycerol（TG）,low density lipoprotein cholesterol（LDL-C）, high density lipoprotein cholesterol （HDL-C） and homoeysteine （Hcy） were lower than the control group,there was statistical difference between two groups ; Serum Hcy, TC, TG and LDL were positively correlated with SLEDAI score. Serum Hey level was positively correlated with TC,TG and LDL; Heart function between the two groups had no obvious change.Conclusion The patients with systemic lupus erythematosus pregnancy using hydroxychloroquine can significantly improve lipid metabolism, and has little effect on cardiac function. So that patients with SLE using hydroxychloroquine were beneficial during pregnancy, and can effectively improve the glucose and lipid metabolism,reduce the risk of cardiovascular disease and SLEDAI score.
