认知心理干预对双相障碍抑郁患者的疗效

The clinical effect of cognitive-behavioral psychological intervention on patients with bipolar disorder

目的探讨认知心理干预对双相障碍抑郁患者的临床疗效。方法采用随机数字法将符合《美国精神病学会精神障碍诊断与统计手册（第三版）》制定的诊断标准的82例双相障碍患者分为观察组42例和对照组40例，观察组采用认知心理干预治疗。采用世界卫生组织生存质量测定量表（WHOQOL—BREF）评定临床症状及生存质量、采用汉密尔顿抑郁量法评定临床疗效。结果干预8周后观察组与对照组总有效率为95．2％和82．5％，差异有统计学意义（P〈0．05）。两组干预后WHOQOL—BREF4个领域评分与干预前比较差异有统计学意义（P〈0．05），两组间比较差异也有统计学意义（P〈0．05）。结论认知心理干预可以对双相障碍抑郁患者产生较好的恢复作用、增强康复效果和提高生存质量。

Objective To evaluate the clinical effect of cognitive-behavioral psychological intervention on patients with bipolar disorder. Methods 82 patients with bipolar disorder conforming to the diagnostic criteria of Diagnostic and Statistical Manual of Mental Disorders （DSM- Ⅲ ） were divided into observation group （42 cases） and control group （40 cases） with the random number table method. The observation group was treated with cognitive-behavioral psychological intervention. World Health Organization on Quality of Life Brief Scale （WHOQOL-BREF） was used as the tool of assessment for clinical symptoms and quality of life, and Hamilton Depression Scale （HAMD） for the clinical efficacy. Results After the intervention for 8 weeks, the total effective rates of observation group and control group were respectively 95.2% and 82.5%, with statistically significant difference （P〈0.05）. There were statistically significant differences in the scores of 4 fields of WHOQOL-BREF between two groups before and after intervention （P〈0.05）, and there were statistically significant differences between two groups after intervention （P〈0.05）. Conclusion Cognitive-behavioral psychological intervention could enhance recovery function and improve the quality of life of patients with bipolar disorder.
