脊柱骨折行内固定结合椎体成形术治疗的效果与临床优势分析

Effect and clinical advantages of internal fixation combined with vertebroplasty in the treatment of spine fracture

目的研究并探讨脊柱骨折患者行内固定结合椎体成形术的临床效果。方法选取2013年1月~2016年5月南京医科大学第二附属医院收治的100例脊柱骨折患者作为前瞻性研究对象,采取计算机数字分组法将患者分为对照组和观察组,每组50例,对照组患者实施单纯内固定术治疗,观察组患者实施内固定联合椎体成形术,比较两组患者的临床治疗优良率、伤椎前后缘高度比、后凸角度、Frankel功能分级情况。结果观察组患者的临床治疗优良率为94.0%,明显高于对照组76.0%(P<0.05);手术后两组患者的伤椎前缘高度比、伤椎后缘高度比、后凸角度均明显改善,差异均具有统计学意义(P<0.05);术后观察组患者的Frankel功能分级正常率较手术前明显增多(P<0.05),且高于术后对照组(P<0.05)。结论采用内固定联合椎体成形术治疗脊柱骨折临床疗效显著,可有效促使患者脊柱椎体高度、活动功能恢复正常,改善预后。

Objective To study and explore the clinical effect of internal fixation combined with vertebroplasty in patients with spinal fractures. Methods Totally 100 cases of spinal fracture,who were treated in the Second Affiliated Hospital of Nanjing Medical University from Jan. 2013 to May 2016 ,were selected and were divided into control group and observation group by computer random number grouping ,with 50 cases in each group. The patients in the control group were treated with simple internal fixation,while the patients in the observation group were treated with internal fixation combined with vertebroplasty. The excellent and good rate of clinical treatment, anteroposterior height ratio, kyphotic angle, and Frankel function classification were compared between the two groups. Results The excellent and good rate of clinical treatment in the observation group was 94% ,which was significantly higher than that in the control group （ 76% ,P 〈 0. 05 ）. The height ratio of the anterior margin of the injured vertebrae, the posterior margin height and the kyphotic angle were significantly improved in both groups and also had statistical differences between the two groups （ P 〈 0. 05 ）. The Frankel function recovery rate in the observation group was higher than that in the control group （ P 〈 0. 05 ） , and the difference was statistically significant （ P 〈 0. 05 ）. Conclusion The clinical effect of internal fixation combined with vertebroplasty in the treatment of spinal fractures is significant ,which can effectively promote the vertebral height of the vertebral body and restore the normal function and improve the prognosis.
