原发性高血压合并2型糖尿病患者的心电图分析

ANALYSIS OF ELECTROCARDIOGRAM IN PATIENTS WITH ESSENTIAL HYPERTENSION COMBINED WITH TYPE 2 DIABETES MELLITUS

目的:研究原发性高血压合并2型糖尿病患者的心电图特征。方法:选择原发性高血压合并2型糖尿病患者87例作为研究组,以同期入院单纯原发性高血压患者91例为对照组,分析两组患者的心电图特点。结果:一般临床资料显示,饮酒、家族史,两组差别有统计学意义(P<0.05);而性别、年龄、体重指数、吸烟等指标,差别无统计学意义(P>0.05)。研究组心电图异常率为75.9%(66/87例),对照组为51.6%(47/91例),差异具有统计学意义(P<0.01)。研究组ST-T改变者36例(41.4%)、传导异常19例(21.8%)、病理Q波7例(8.0%),对照组分别为17例(18.7%)、3例(3.3%)、1例(1.1%),差异均有统计学意义(P<0.05);而房室肥大、期前收缩、窦性心动过速/窦性心动过缓、肢体导联低电压,两组差别无统计学意义(P>0.05)。结论:原发性高血压合并2型糖尿病患者较原发性高血压心电图异常率高,主要体现为ST-T改变、传导异常、病理Q波增多。

Objective： To study characteristics of electrocardiogram in patients with essential hypertension combined with type 2 diabetes mellitus. Methods： Eighty-seven patients with essential hypertension combined with type 2 diabetes mellitus were into the study group, and ninety-one patients with essential hypertension as the control group. The characteristics of electrocardiogram was analyzed between the two groups. Results：General clinical data showed that there was statistically significant difference in alcohol drinking and family history in the two groups （P〈0.05）, without significant difference in gender, age, body mass index, smoking （P〉0.05）. The abnormal rate of electrocardiogram was 75.6% （66/87 cases）, and 51.6 % （47/91 cases） in control group （P〈0.01）. In study group, there were 36 cases （41.4%） of ST-T changes, 19 cases of abnormal conduction （21.8%）, 7 cases of pathological Q wave （8%）, and 17 cases, 3 cases and 1 case respectively in control group （P〈0.05）. There was no significant difference in the incidence of atrioventricular hypertrophy, premature beats, sinus tachycardia/sinus bradycardia and limb lead low voltage in the two groups （P〉0.05）. Conclusion： There is a higher incidence of abnormal ECG in patients with essential hypertension combined with type 2 diabetes mellitus than that of essential hypertension patients, which is mainly manifested as ST-T change, conduction abnormality and pathological Q wave.
