正常人寰枢椎相对旋转的CT研究与旋转固定的对照分析

The investigation of the relative rotation of C_1 and C_2 in functional position by CT and compared with AARF

目的 :研究正常人功能位寰枢椎相对旋转角度的变化并与旋转固定的患者比较分析 ,为寰枢关节旋转性固定的诊断提供帮助。方法 :82例正常人组与 18例寰枢关节旋转性固定患者组行中立位CT扫描 ,然后行左、右旋转 2 5°～ 4 5°功能位扫描 ,测量功能位寰枢椎相对旋转角度 ,同时对比观察寰齿前间隙与齿突与环椎侧块间隙变化情况。结果 :寰枢椎相对旋转角度在正常人组不同年龄阶段存在差异 ,4 0岁以前与 4 0岁以后的差别有统计学意义 (P <0 .0 5 ) ,旋转固定患者组与正常人组各年龄段间有显著差异 (P <0 .0 5 ) ,正常人组与旋转固定患者组功能位比较 ,寰齿前间隙的改变有意义 (P <0 0 5 ) ,齿突与环椎侧块间隙的改变无意义 (P >0 .0 5 )。结论 :测量功能位寰枢椎相对旋转角度 ,分析寰齿前间隙的改变 。

Objective:To investigate the change of the relative rotation angle of C 1 on C 2 in the functional position using CT.Nomal group and atlantoaxial rotatory fixation(AARF) groups were compared and analysed.The purpose of the study is to help the dianosis of the AARF.Methods:The atlanto axial vertibrae of 82 nomal subjects and 18 cases of AARF were scanned in neutral position first,then with the head roating 25°to 45° the right or left respectively.In two groups the relative rotatory angle of C 1 on C 2 in the functional position were measured,the changes of ADI and LADS were compared as well.Results:There were differences in the relative rotatory angle of C 1 on C 2 of different age in normal groups.There was statistical significance between the case of less 40 and beyond 40 years old.Compared ARRF groups with normal ones,there were some statistical difference,which include the change of ADI and the LADS.Conclusion:Measuring and analysing the relative rotation angle of C 1 on C 2 in functional position by CT,and the change of ADI would be helpful for the diagnosis of ARRF.
