原发性垂体柄占位性病变的MRI诊断及治疗(附5例报告)

目的 :探讨垂体柄占位性病变的MRI表现及治疗方法。方法 :回顾性分析 5例垂体柄占位性病变的临床及MRI资料 ,其中男 2例 ,女 3例 ,平均年龄 2 3岁。结果 :MRI的特征性表现为垂体柄球形增粗 ,3例生殖细胞瘤T1、T2 等信号 ,增强扫描病变强化均匀一致 ,其中 1例经手术病理证实 ,3例均行放射治疗 ,疗效明显 ;2例Rathke’s囊肿 ,T1等、低信号 ,T2 高信号 ,增强扫描仅示包膜环形强化 ,均行手术切除病理证实。结论 :MRI可以清楚的显示原发性垂体柄占位病变形态 ,有别于继发性病灶 ;

Objective:To study the MR features and diagnostic value of the pituitary stalk space occupying lesions, explore the operation methods.Methods:5 patients with pituitary stalk space occupying lesions include 2 male, 3 female, average 23 years old, are performed MR examination, and 3 cases are performed right pterional approach operation after which make pathology examination.Results:3 cases are germinoma, MR show equal signal on T 1 and T 2, the lesions are enhanced completely, 1 case is given operation and 2 cases are given radiation therapy. 2 cases are Rathke's cyst, which are given operation. MR show equal and low signal on T 1, high signal on T 2, the lesion envelope is enhanced after reinforcement.Conclusion:MR can clearly show shapes, sizes of the pituitary stalk space occupying lesions. Germinoma is sensitive to radiation therapy.
