Application of 1.5T Magnetic Resonance Imaging Scan in Differential Diagnosis on Temporal Jaw Joint Disorder Syndrome

摘要 目的:探讨1.5T磁共振(MRI)扫描下在鉴别诊断颞颌关节紊乱综合征中的应用。方法:选取2013年9月-2016年9月期间本院确诊治疗的颞颌关节紊乱综合征患者30例(48个颞颌关节病变),同期选取体检中心20例健康人员(40个颞颌关节),所有患者均给予1.5T的MRI扫描和X线扫描检查,以手术检查或病理学检查或关节造影和内镜检查等结果为对照,统计分析所有研究对象MRI、X线检查颞颌关节情况。结果:X线扫描检查可显示关节间隙和骨质改变,但对关节盘显像不清晰,其鉴别诊断颞颌关节紊乱综合征的敏感度、特异度、准确度分别为83.33%、94.23%、89.00%,MRI扫描检查可清晰显示关节间隙和骨质改变及关节盘及其相关组织的结构状态,其鉴别诊断颞颌关节紊乱综合征的敏感度、特异度、准确度分别为95.83%、100%、98.00%,前者的敏感度、准确度明显低于后者,有统计学差异(P<0.05),但两者特异度基本相同,比较无显著差异(P>0.05)。结论:1.5T的MRI扫描在鉴别诊断颞颌关节紊乱综合征中具有更为良好的敏感性和准确性,且具有无创、图像清晰、操作简单、无辐射等特点,值得临床作进一步推广。 Objective: To discuss the application of 1.5T magnetic resonance imaging ( MHI ) scan in differential diagnosis on temporal jaw joint disorder syndrome.Method : 30 patients with temporal jaw joint disorder syndrome ( 48 the jaw joints lesions ) were selected from Septemher 2013 to September 2016 in our hospilal, 20 cases of healthy persons ( 40 temporal jaw joints ) were chose from a medical center at the same time, all patients were given 1.5T MRI scan and X Line scan to check or joint imaging and endoscopic examination or surgery pathology inspection result for reference, statistical analyzed all studies MRI, X-ray objeel the jaw joints. Result: X-ray scan showed that, joint clearance and bone changes, but the joints imaging was not clear, the differential diagnosis of temporal jaw joint disorder syndrome sensitivity, specific, accuracy degree were respectively 83.33%, 94.23% and 89.00%.MRI so, an showed, it could clearly show the joint and bone and joint plate and its relatedstate of the structure of the organization, its differential diagnosis of the temporal jaw joint disorder syndrome sensitivity, specific and accuracy degrees were respectively 95.83%, 100% and 95.83%, the former sensitivity and accuracy were significantly lower than the latter, there were statistically significant ( P〈0.05 ), but the specificity of the two basically the same, there was no significant difference (P〉0.05) .Conclusion: 1.5T MRI scan in differential diagnosis of the temporal jaw joint disorder syndrome has more good sensitivity and accuracy, and it has a noninvasive, image clear, simple operation, no radiation and other characteristics, it's worth for further clinical promotion.
