糖化白蛋白、同型半胱氨酸与脂蛋白a在妊娠高血压诊断中的意义

The correlation relationship of glycated albumin and homocysteine and lipoprotein a in serum in the clinical application of pregnancy induced hypertension

目的探讨糖化白蛋白(GA),人血清同型半胱氨酸(Hcy)与脂蛋白a(Lpa)在妊娠高血压临床诊断中的意义。方法测定50例健康体检组、正常妊娠50例和159例妊娠高血压患者血清中GA、Lpa和Hcy水平,并进行分析对比。结果健康对照组与正常妊娠组的GA、Hcy和LPa差异无统计学意义,妊娠高血压组显著高于健康对照组和正常妊娠组(P<0.05);GA、Hcy和LPa是妊娠高血压的独立相关因素(P<0.05);GA、Hcy和LPa联合检测的灵敏度、阴性预测值和约登指数显著高于单个指标(P<0.05)。结论 GA、Lpa和Hcy异常代谢均与妊娠高血压有关,三者联合测定,可提高对妊娠高血压诊断的阳性率,有助于临床对疾病的诊断和判断预后。

Objective To investigate the correlation of GA,Lpa and HCY in serum in clinical application of pregnancy in- duced hypertension. Methods GA,Lpa and HCY in the serum of 50 cases of healthy group and 50 cases of healthy pregnancy group and 159 patients with pregnancy induced hypertension were detected, and statistical analysis was progressed. Results There was no significant difference in GA ,Lpa and HCY between health control group and healthy pregnancy group. The pregnancy induced hyperten- sion group was significantly higher than that in healthy control group and normal pregnancy group. GA, HCY and LPa were independent factors related to pregnancy induced hypertension （ P 〈 0. 05）. The sensitivity, negative predictive value and Youden index in combined detection of GA, HCY and LPa were significantly higher than that of the single index. Conclusion Abnormal metabolism of GA and HCY and Lpa are associated with pregnancy induced hypertension. Combined determination can improve the diagnosis of pregnancy in- duced hypertension, which is helpful for clinical diagnosis and prognosis.
