上肢PICC置管在隐匿上腔静脉综合征放化疗期间的应用与观察

Nursing group intervention in patients with percutaneous tracheotomy rotary expansion severe emergency application research

目的:比较隐匿上腔静脉综合征选择不同静脉通路的临床应用和效果,选择安全舒适度较高的血管通路,帮助患者完成放化疗治疗。方法:选取2015年1月至2016年12月隐匿上腔静脉压迫的患者72例,按照患者意愿分成观察组37例,对照组35例。观察组行上肢PICC置管,对照组采用经下肢开放静脉通路。观察比较两组患者在病情进展、静脉炎、药物外渗、导管相关血栓、导管血流感染以及治疗期间舒适度等方面的异同。结果:两组间病情进展无明显不同(P>0.05),但观察组在静脉炎、药物外渗、导管相关血栓以及血流感染方面的发生明显低于对照组,观察组治疗期间舒适度明显高于对照组,差异有统计学意义(P<0.05)。结论:隐匿上腔静脉综合征患者选择血管通路时,综合全面分析结果,选择经上肢PICC置管临床效果更佳。

Objective ：Comparison of occult superior vena cava syndrome to choose the clinical application of different venous access and the effect of choice of safe and comfortable vascular access, to help patients complete radiotherapy and chemotherapy. Methods： seventy-two patients with occult superior vena cava compression from January 2015 to December 2016 were divided into observation group （37 cases） and control group （35 cases） according to the patients wishes. The rats in the observation group were treated with PICC, and the control group was treated with open venous access through the lower limbs. To observe the similarities and differences between the two groups in patients with disease progression, phlebitis, catheter-related throm- bosis, catheter blood flow infection and comfort during treatment. Results：There was no significant difference between the two groups （P 〉0. 05）. The incidence of phlebitis, drug extravasation, catheter-related thrombosis and blood flow in the observa- tion group was significantly lower than that in the control group. The difference was statistically significant （P 〈 0. 05）. Conclusion.Occlusion of superior vena cava syndrome patients choose vascular access, comprehensive comprehensive analysis of the results, select the upper limb PICC catheter clinical effect is better.
