摘要
目的:了解静脉导管未闭新生儿血浆前列腺素E_2的表达水平,并探讨新生儿静脉导管未闭机制。方法:选择确诊为静脉导管未闭的新生儿95例作为病例组,并依据胎龄分为早产组(45例,28≤胎龄<37周)与足月组(50例,37≤胎龄<42周);选择确诊为静脉导管闭合的足月新生儿50例作为对照组。于出生后2~3天应用彩色多普勒超声检测病例组新生儿静脉导管内径、平均血流速度并计算血流量;用酶联免疫吸附法(ELISA)测定各组新生儿血浆前列腺素E_2水平。结果:早产组的静脉导管血流量大于足月组,两组间差异有统计学意义(P<0.01);早产组的血浆前列腺素E_2水平高于足月组及对照组,足月组的血浆前列腺素E_2水平高于对照组,三组间差异有统计学意义(P<0.01)。结论:静脉导管未闭新生儿血浆前列腺素E_2表达水平增高,而且血浆前列腺素E_2水平越高,静脉导管血流量越大,因此血浆前列腺素E_2表达水平增高会影响新生儿静脉导管闭合。
Objective To investigate the level of plasma prostaglandin E2 in neonates with patent ductus venosus and the mechanism of neonatal ductus venous. Method 95 newborns diagnosed as patent ductus venous were divided into Preterm Group (45 cases,gestational age from 28 to 37 weeks) and Term Group (50 cases,gestational age from 37 to 42 weeks). 50 cases of term neonates diagnosed as ductus venous closure were selected as control Group. The ductus venosus diameter and blood flow ve-locity of the neonates were detected by color Doppler ultrasound, and the blood flow volume was also calculated. The plasma pros-taglandin E2 levels were measured by enzyme - linked immunosorbent assay (ELISA) in all groups. Results Preterm Group had higher blood flow volume of ductus venous than Term Group,the difference between the two groups was statistically significant (P 〈0. 01). The plasma prostaglandin E2 level in Preterm Group was higher than that in Term Group and Control Group,the lev-el of prostaglandin E2 in the Term Group was higher than that in the Control Group,and the differences among the three groups were statistically significant( P 〈0.01). Conclusion Plasma prostaglandin E2 level in neonates with patent ductus venosus in-creased, and the higher the plasma prostaglandin E2 level,the greater the blood flow of the ductus venosus. Therefore,the increase of plasma prostaglandin E2 may affect the closure of neonatal ductus venosus.
