综合脱机指数和用力呼吸指数对预测机械通气患者脱机能力的作用 被引量:1

摘要 目的:探讨综合脱机指数(integrated weaning index,IWI)和用力呼吸指数(respiratory exertion index,CORE)对重症监护病房(intensive care unit,ICU)患者脱机能力的预测价值。方法:选取入住ICU的机械通气患者83例为研究对象,计算出患者IWI和CORE,统计患者脱机成功率,比较脱机成功患者与失败患者的IWI和CORE变化,分析IWI和CORE对判断患者脱机能力的预测价值。结果:83例患者中,64例脱机成功,占77.11%,19例脱机失败,占22.89%;脱机成功患者自主呼吸实验前IWI为(85.32±7.48)、CORE为(6.83±1.28),分别高于脱机失败患者的IWI(56.58±3.16)(t=24.291,P=0.000)以及CORE(3.47±0.63)(t=11.018,P=0.000)。结论:运用IWI及CORE预测患者脱机能力,具有一定的临床参考价值,值得推广。 Objective:To investigate the predictive value of integrated weaning index (IWI) and respiratory exertion index (CORE) for wea- ning ability of patients in intensive care unit (ICU). Methods:83 cases of mechanical ventilation patients in ICU were selected. IWI and CORE were calculated and the success rate of weaning was analyzed. The changes of IWI and CORE in successful weaning patients and unsuccessful wea- ning patients were compared to investigate their predictive value. Results:In the 83 patients, there were 64 cases successful in weaning, accounting for 77.11%, and 19 cases of failure, accounting for 22.89 %. Before spontaneous breathing test, IWI and CORE in successful weaning patients were (85.32± 7.48) and (6.83± 1.28) respectively, which were significantly higher than those in unsuccessful weaning patients ( (56.58 ± 3.16) and (3.47 ± 0.63) respectively) ( t =24.291, P =0.000; t =11.018, P =0.000). Conclusion:IWI and CORE can be used to predict the weaning ability of patients in ICU, which is worthy of clinical promotion.
