枸橼酸咖啡因联合加温湿化高流量鼻导管通气治疗早产儿呼吸暂停的疗效 被引量:2

摘要 目的:探讨枸橼酸咖啡因联合加温湿化高流量鼻导管通气治疗早产儿呼吸暂停的临床效果。方法:选取2016年1月至2017年2月间新生儿重症监护病房收治的呼吸暂停新生儿160例作为研究对象,随机分为观察组和对照组各80例,对照组患儿给予氨茶碱静脉注射治疗,观察组患儿给予枸橼酸咖啡因联合加温湿化高流量鼻导管通气治疗,观察两组患儿的临床疗效。结果:观察组患儿72 h内发生呼吸暂停次数、呼吸暂停消失时间、总用氧时间以及需要有创通气的例数等指标均低于对照组(P<0.05),发生腹胀、心率加快、喂养不耐受、胃潴留以及支气管肺发育不良的情况均低于对照组(P<0.05)。结论:枸橼酸咖啡因联合加温湿化高流量鼻导管通气治疗可以有效减轻早产儿呼吸暂停引起的缺氧,减少有创通气并发症和用氧时间,减少对患儿机体的危害,安全有效,可推广使用。 Objective :To investigate the clinical effect of caffeine citrate combined with heating and moisturizing high - flow nasal catheter ventilation in the treatment of apnea in premature infants. Methods: 160 cases of apnea neonates admitted to the neonatal intensive care unit between January 2016 and February 2017 were randomly divided into study group and control group, 80 cases in each group. The neonates in study group were treated with caffeine citrate combined with heated and humidified high - flow nasa/catheter ventilation, and those in control group were treated with aminophylline intravenous injection. The clinical efficacy in the two groups was observed. Results : The frequency of apnea, apnea time, total oxygen consumption and the number of cases requiring invasive ventilation within 72 h in study group were significantly lower than those in control group( P 〈 0.05 ). The incidences of abdominal distension, accelerated heart rate, feeding intolerance, gastric retention and bronchopulmonary dysplasia were significantly lower than those in control group ( p 〈 0.05 ). Conclusion : Citrate caffeine combined with heating and moisturizing high -flow nasal catheter ventilation can effectively reduce the hypoxia caused by apnea in premature in/ants, reduce the complications of ventda- tion and oxygen use time, and reduce the harm to ehildren's body. With its safety and effectiveness, it is worth being promoted.
