摘要
目的探讨早期肠内营养联合益生菌对减少呼吸机相关性肺炎(VAP)的价值分析。方法回顾性分析2013年2月至2015年9月收治机械通气患者98例,研究组采用早期肠内营养联合益生菌,对照组仅用早期肠内营养,两组各49例,对两组VAP发生率、30 d病死率、住院时间、机械通气时间进行统计学分析。结果研究组患者VAP发生率低于对照组(χ~2=6.078,P<0.05);撤机后对照组再次发生VAP多于研究组(χ~2=4.436,P<0.05),差异均有统计学意义;研究组住院时间(8.90±2.43)d少于对照组(12.09+3.67)d;研究组机械通气时间(53.8±3.90)h也少于对照组(62.90±4.90)h,差异具有统计学意义(P<0.05)。结论早期肠内营养联合益生菌可以显著减少VAP发生,缩短ICU病房住院时间及机械通气时间,减少患者住院费用,值得临床上广泛使用。
Objective To explore the clinical significance of early enteral nutrition combined with probiotics on reducing ventilator associated pneumonia (AVP). Methods From February 2013 to September 2015 in our hos- pital, 98 patients with mechanical ventilation were selected and they were randomly divided into 2 groups, 49 cases in each group. The observation group was treated with early enteral nutrition combined with probiotics, in the control group, just treated with early enteral nutrition. The incidence of VAP, 30-day mortality, length of hospital stay and duration of mechanical ventilation in two groups were conducted statistical analysis. Results The VAP incidence of patients in the observation group was significantly lower than that of the control group (X2= 6. 078, P〈0. 05). After cure of infection, the control group to VAP infection was higher than that of the observation group (X2 = 4. 436, P〈0. 05), the comparison showed significant difference. The observation group of patients in ICU hospital stay (8.90+2.43) ds was significantly less than that of the control group ( 12.09±3.67) ds. Patients in the observation group ( 53.8 ± 3.90) h were significantly less than those in the control group (62. 90±4. 90) h with the time of breathing machine, and the comparison showed significant difference (P〈0. 05). Conclusion Early enteral nutrition combined with probiotics can significantly reduce the incidence of ventilator associated pneumonia, shorten the length of stay in the ICU ward and the use of breathing machine, reduce the cost of hospitalization, it is worthy to be widely used in clinical practice.
