PD-1/PD-L1在非小细胞肺癌中表达的生物学作用及临床意义 被引量:4

摘要 目的研究PD-1/PD-L1及EGFR在非小细胞肺癌中的表达和相关性。方法收集我院96例非小细胞肺癌患者的病理标本和临床资料,使用免疫组化检测PD-1/PD-L1的表达情况,PCR检测EGFR的表达情况。使用Logistic回归分析评价PD-1/PD-L1和EGFR的相关性。结果吸烟患者的PD-1阳性率为76.7%(23例),显著高于不吸烟患者的21.2%(14例)(P<0.05)。女性患者的PD-L1阳性率为82.0%(41例),显著高于男性的45.7%(21例)(P<0.05)。腺癌患者的PD-L1阳性率为78.3%(47例),显著高于鳞癌的41.7%(15例)(P<0.05)。EGFR突变型患者的PD-L1阳性率为91.9%(34例),显著高于野生型的47.5%(28例)(P<0.05)。结论 PD-1在吸烟患者中高表达;PD-L1与EGFR呈正相关,且在女性、腺癌患者中高表达,PD-L1与EGFR基因表达的关系可能为个体化免疫治疗联合靶向治疗提供新的依据。 Objective To study the PD-1/PD-L1 expression in non-small cell lung cancer and the correlation with EGFR expression. Methods Pathologic specimens of 96 cases of patients with non-small cell lung cancer were selected and their clinical datas were collected. PD-1/PD-L1 expression were detected by immunohistochemical and PCR for EGFR expression. Logistic regression analysis was used to evaluated the correlation between PD-1/PD-L1 and EGFR. Results The positive rate of PD-1 of smoking patients was 76.7% (23 cases), significantly higher than 21.2% (14 cases) of non-smoking patients (P〈0.05). The positive rate of PD-L1 of female patients was 82.0% (41 cases), significantly higher than 45.7% (21 cases) of male patients (P〈0.05). The positive rate of PD-L1 of adenocarcinoma was 78.3% (47 cases), significantly higher than 41.7% (15 cases) of squamous cell carcinomas (P〈0.05). The positive rate of PD-L1 of EGFR mutation type was 91.9% (34 cases), significantly higher than 47.5% (28 cases) of wild type (P〈0.05). Conclusions Smoking patients have higher PD-1 expression. PD-L1 is positively correlated with EGFR. Female patients and adenocarcinoma patients have high PD-L1 expressiom The correlation between PD-L1 and EGFR may provide new evidence for individual immunotherapy combined with targeted therapy.
