踝关节骨折术前分型和术后评估中采用螺旋CT三维重建技术的临床价值

The Clinical Value of Three Dimensional Reconstruction of Spiral CT in Preoperative Classification and Postoperative Evaluation of Ankle Fracture

目的探讨螺旋CT三维重建技术在踝关节骨折术前分型和术后评估方面的应用效果。方法回顾性分析2016年1—12月在我院住院治疗的110例踝关节骨折患者临床病例资料。结果与术中各分型相比,术前CT三维重建对PER、PAB和SER、SAB型的Kappa值分别为0.78、0.82、0.80、0.77,一致性良好;术后行螺旋CT三维重建对踝关节分型解剖复位和内固定置入适宜检出率明显优于DR检查,两者比较差异具有统计学意义(P<0.05)。结论螺旋CT三维重建技术在踝关节骨折术前分型和术后评估方面准确可行。

Objective To evaluate the effect of three-dimensional reconstruction of spiral CT in the classification and postoperative evaluation of ankle fracture. Methods The clinical data of 110 patients with ankle fracture treated in our hospital from January to December 2016 were analyzed retrospectively. Results Compared with the intraoperative results, the preoperative CT, three-dimensional reconstruction of PER, PAB and SER, SAB type Kappa values were 0.78, 0.82, 0.80 and 0.77, both of which were in good agreement. After the operation, the three-dimensional reconstruction of spiral CT was better than the DR in the classification, reduction and internal fixation of ankle joint, the difference was statistically significant （P〈0.05）. Conclusion The three-dimensional reconstruction of spiral CT is accurate and feasible in the preoperative classification and postoperative evaluation of ankle fracture.
