PCNA、CD34在膝关节骨关节炎骨赘中的表达意义及相关性探析

摘要 目的探析膝关节骨关节炎骨赘中CD34、PCNA的表达水平及临床意义。方法临床选择2015年4月—2016年8月本院收治的膝关节骨关节炎骨赘患者50例为观察组,同期选择体检健康者50例为研究对照,膝关节骨关节炎骨赘患者进行骨赘清理与关节镜下有限清理联合疗法,应用免疫化学法检测两组的骨组织中CD34、PCNA的表达情况,比较观察组治疗前与治疗后的骨组织中CD34、PCNA的阳性表达情况;比较观察组与对照组的骨组织中CD34、PCNA的阳性表达情况;比较观察组不同临床分期的骨组织CD34、PCNA的阳性表达情况,观察临床分期与两种指标的关系。结果观察组治疗后的膝关节骨组织中的CD34、PCNA的阳性表达情况显著低于治疗前,差异有统计学意义(P<0.05);观察组的膝关节骨组织中CD34、PCNA的阳性表达情况显著高于对照组,差异有统计学意义(P<0.05);观察组的临床分期IIb、III期的骨组织CD34、PCNA的阳性表达情况显著高于I、IIa期,差异有统计学意义(P<0.05)。临床分期与骨组织CD34、PCNA阳性表达呈明显正相关,临床分期越高,骨组织CD34、PCNA阳性表达越多。结论膝关节骨关节炎骨赘中CD34、PCNA多呈阳性表达,与膝关节骨关节炎骨赘的临床分期密切相关。 Objective To investigate the expression level and clinical significance of CD34 and PCNA in osteophyte of knee joint and osteoarthritis. Methods The clinical data of 50 patients with osteophyte of knee joint and osteoarthritis treated in our hospital from April 2015 to August 2016 were selected as the observation group, and 50 cases of healthy volunteers were selected as the control group. The combined therapy of osteophyte clearance and limited debridement under arthroscopy were carried out among patients with osteophyte of knee joint and osteoarthritis. The expressions of CD34 and PCNA in bone tissue of two groups were tested by applying immunochemistry to compare the positive expression of CD34 and PCNA in the bone tissue before and after the treatment in the observation group, to compare the positive expression of CD34 and PCNA in the bone tissue between observation group and control group the positive expression of CD34 and PCNA in bone tissue of different clinical stages in the observation group was compared, and to observe the relationship between clinical stages and two indexes. Results After treatment, the positive expression of CD34 and PCNA in the knee joint bone tissue of the observation group was significantly lower than that before the treatment, and the difference was statistically significant (P〈0.05); The positive expression of CD34 and PCNA in the knee joint bone tissue of the observation group was significantly higher than that of the control group and the difference was statistically significant (P〈0.05); The positive expression of CD34 and PCNA of the bone tissue in the clinical stages of lib and Ⅲ were significantly higher than those of I and IIa, and the difference was statistically significant (P〈0.05). The clinical stage was positively related to the positive expression of CD34 and pCNA in bone tissue, and the higher the clinical stage was, the more positive expression of CD34 and PCNA in bone tissue was. Conclusion The expression of CD34 and PCNA in the osteophyte of knee joint and osteoarthritis is mostly positive and is closely related to the clinical stage of osteophyte of knee joint and osteoarthritis
