人工股骨头置换术与全髋关节置换术治疗股骨颈骨折的临床效果比较研究

Clinical effect comparison of artificial femoral head replacement surgury and total hip arthroplasty surgury in the treatment of femoral neck fracture

目的比较人工股骨头置换术与全髋关节置换术治疗股骨颈骨折的临床效果。方法选取海西州人民医院2015年10月—2016年10月收治的股骨颈骨折的患者120例,根据治疗方法的不同分为A组和B组,各60例。A组患者采用人工股骨头置换术治疗,B组患者采用全髋节置换术治疗。比较两组患者的手术时间、术中出血量、术后下床活动时间及临床疗效,并观察两组患者并发症发生情况。结果 B组患者手术时间长于A组,术中出血量多于A组,术后下床活动时间短于A组(P<0.05)。B组患者治疗优良率高于A组,并发症发生率低于A组(P<0.05)。结论人工股骨头置换术和人工全髋节置换术对治疗股骨颈骨折各有优缺点,因此在临床治疗中,临床医生可根据患者自身情况选择合适的置换治疗方法,才可最大限度体现其治疗价值。

Objective To compare the clinical effect of artificial femoral head replacement surgury and total hip arthroplasty surgury in the treatment of femoral neck fracture.Methods A total of 120 cases of patients with femoral neck fracture were selected from October 2015 to October 2016 in People ＇s Hospital of Haixi Prefecture,which were divided into A group and B group according to different methods of treatment,60 cases in each group.A group was treated with artificial femoral head replacement surgury,B group was treated with total hip arthroplasty surgury.The operation time,intraoperative blood loss,postoperative bed activity time and clinical effect were compared between the two groups,and the complications were observed.Results B group of operation time was longer than A group,intraoperative blood loss was more than A group,postoperative bed activity time was shorter A group（P〈0.05）.The B group of good rate was higher than A group,rate of complications was lower than A group（P〈0.05）.Conclusion Artificial femoral head replacement surgury and total hip arthroplasty surgury have their advantages and disadvantages in the treatment of femoral neck fractures,so in the clinical treatment,clinicians should choose the appropriate replacement therapy based on the patients＇ situation,so as to greatest reflect the value of its treatment.
