右美托咪定在腹腔镜胆囊切除术麻醉中的应用效果

Application effect of dexmedetomidine in anesthesia of laparoscopic cholecystectomy

目的探讨右美托咪定在腹腔镜胆囊切除术麻醉中的应用效果。方法选取灵寿县医院2015—2016年收治的行腹腔镜胆囊切除术患者110例,根据麻醉方法不同分为对照组和观察组,各55例。术前10 min,对照组患者静脉注射0.9%氯化钠溶液,观察组患者予以右美托咪定,两组患者均采用丙泊酚维持麻醉。比较两组患者入室、切皮和拔管时的血压[收缩压(SBP)和舒张压(DBP)]、心率(HR)及丙泊酚用量、术后拔管时间及术后躁动发生率。结果入室时两组患者SBP、DBP及HR比较,差异无统计学意义(P>0.05);切皮、拔管时观察组患者SBP、DBP及HR水平低于对照组(P<0.05)。观察组患者丙泊酚用量少于对照组,术后拔管时间短于对照组,术后躁动发生率低于对照组(P<0.05)。结论右美托咪定在腹腔镜胆囊切除术麻醉中的应用效果确切,可有效稳定患者血压和心率,且安全性较高。

Objective To discuss the application effect of dexmedetomidine in anesthesia of laparoscopic cholecystectomy.Methods A total of 110 cases of patients underwent laparoscopic cholecystectomy with anesthesia were selected from2015 to 2016 year in Lingshou County Hospital,which were divided into control group and observation group according to different methods of anesthesia,55 cases in each group.Before 10 minutes of laparoscopic cholecystectomy,control group was given intravenous injection of 0.9% sodium chloride solution,observation group was given dexmedetomidine,both groups were maintained anesthesia with propofol.The blood pressure（SBP,DBP）,HR at the time of entering operating room,skin incision and extubation,and the dosage of propofol,postoperative extubation time and the incidence of restlessness were compared between the two groups.Results At the time of entering operating room,there was no significant differences of SBP,DBP or HR between the two groups（P〉0.05）;at the time of skin incision and extubation,the observation group of SBP,DBP and HR were lower than control group（P〈0.05）.The observation group of dosage of propofol was less than control group,postoperative extubation time was shorter than control group,the incidence rate of restlessness was lower than control group（P〈0.05）.Conclusion Dexmedetomidine has an exactly application effect in anesthesia of laparoscopic cholecystectomy,it can make the blood pressure and HR stable of patients,and with high safety.
