松解撬拨闭合复位结合中药熏蒸治疗陈旧性肩关节前脱位临床观察

Clinical Observation of Releasing,Prying and Closed Reduction Combined with Herbal Fumigation for Old Anterior Dislocation of Shoulder Joint

目的:观察松解撬拨闭合复位结合中药熏蒸治疗陈旧性肩关节前脱位的临床疗效。方法:对陈旧性肩关节前脱位患者12例,在全麻或臂丛阻滞麻醉下进行松解撬拨闭合复位,复位成功后经皮克氏针及外用绷带贴胸位固定3周,3周后拔除克氏针并解除外固定,同时配合外用中药熏蒸及肩关节功能锻炼。均观察随访1年,采用Rowe肩关节评分系统进行疗效评定。结果:所有患者均成功复位,其中优3例,良6例,可2例,差1例,优良率75.0%。结论:松解撬拨闭合复位结合中药熏蒸治疗陈旧性肩关节前脱位临床疗效肯定。

Objective： To observe the clinical effect of releasing, prying and closed reduction combined with herbal fumigation for old anterior dislocation of shoulder joint. Methods：Twelve cases of patients with old anterior dislocation of shoulder joint received releasing, prying and closed reduction under general anesthesia or brachial plexus block. After the successful reduction,patients received fixation using Kirschner wires and topical bandage on chest for three weeks. Kirschner wires were extracted and external fixation was removed after three weeks. At the same time, herbal fumigation and functional exercise for the shoulder joint were both given to the patients. All patients were followed up for one year. Rowe shoulder scoring system was used to evaluate the curative effect. Results：Patients all received successful reduction. The results were 3 cases of excellent,6 cases of good,2 cases of fair and 1 case of poor. The excellent-good rate was 75.0%.Conclusion：The therapy of releasing,prying and closed reduction combined with herbal fumigation for old anterior dislocation of shoulder joint has positive effect.
