中西医结合对半月板缝合术后关节功能康复的影响

Effect of Integrated Chinese and Western Medicine Therapy for Joint Rehabilitation after Meniscus Suture

目的:观察中西医结合对半月板缝合术后关节功能康复的影响。方法:选取80例患者,随机分为2组各40例,对照组单纯行关节镜手术治疗及常规功能康复训练护理;治疗组行关节镜手术后,在常规功能康复训练护理基础上,予关节腔内注射透明质酸,并予内服桃红四物汤合五苓散加减治疗。术后2周、术后4周检测疼痛视觉模拟评分法(VAS)、肌肉萎缩程度、关节活动度、Lysholm评分情况。结果:疼痛得分方面、肌肉萎缩程度比较,组内比较,不同时间点2组患者疼痛得分、肌肉萎缩程度比较,差异均有统计学意义(P<0.05);随着治疗时间的延长,患者疼痛得分、肌肉萎缩程度逐渐下降,差异均有统计学意义(P<0.05)。术后2周及4周,观察组疼痛得分、肌肉萎缩程度均低于对照组,差异均有统计学意义(P<0.05)。关节活动范围、Lysholm评分方面比较,组内比较,不同时间点2组患者关节活动范围、Lysholm评分比较,差异均有统计学意义(P<0.05);随着治疗时间的延长,患者关节活动范围、Lysholm评分逐渐增大,差异均有统计学意义(P<0.05)。组间比较,术后2周及4周,观察组关节活动范围、Lysholm评分多于对照组,差异均有统计学意义(P<0.05)。结论:关节腔内注射透明质酸及内服桃红四物汤合五苓散的中西医结合治疗的方法对半月板损伤术后关节功能康复有很好的疗效。

Objective： To observe the clinical effect of integrated Chinese and western medicine therapy for joint rehabilitation after meniscus suture. Methods： Selected 80 cases of patients, and divided them into the two groups randomly,40 cases in each group. Both groups received arthroscopic surgery and conventional rehabilitation training,while the treatment group additionally received injection of hyaluronic acid into articular cavity and oral administration of modified Taohong Siwu tang and Wuling powder. After two weeks and four weeks of treatment,detected visual analogue scale（VAS）,degree of muscular atrophy, range of motion and Lysholm score in both groups. Results：Compared VAS and degree of muscular atrophy in both groups at different time,differences were significant（P〈0.05）. As the course extended,VAS and degree of muscular atrophy were declining gradually,differences both being significant（P〈0.05）. After two weeks and four weeks of treatment,VAS and degree of muscular atrophy in the treatment group were both lower than those in the control group,differences being significant（P〈0.05）. Compared the range of motion and Lysholm scores in both groups at different time,differences were both significant（P〈0.05）. As the treatment prolonged,the range of motion and Lysholm scores were both gradually increasing,differences being significant（P〈0.05）. After two weeks and four weeks of treatment,the range of motion and Lysholm score in the treatment group were both higher than those in the control group, differences being significant（P〈0.05）. Conclusion： The integrated Chinese and western medicine therapy--injection of hyaluronic acid into articular cavity and oral administration of modified Taohong Siwu tang and Wuling powder has good curative effect for joint rehabilitation after meniscus suture.
