基于因子分析的溃疡性结肠炎中医证候规律研究

Study on TCM Syndromes of Ulcerative Colitis on Factor Analysis

目的:采用因子分析研究溃疡性结肠炎的症状组合规律和证候分类及分布规律,为该病的中医辨证分型客观化提供依据。方法:采用临床流行病学的原则和方法,应用因子分析的方法对232例溃疡性结肠炎的症状进行分析研究。结果:因子分析结果显示溃疡性结肠炎基本中医证候为7类:脾胃虚弱证、大肠湿热证、肝郁脾虚证、阴虚肠燥证、脾肾阳虚证、血瘀肠络证、寒热错杂证;得出证型的分布情况,所占比例大小依次为大肠湿热证(22.84%),脾胃虚弱证(17.24%),脾肾阳虚证(15.95%),肝郁脾虚证(13.36%),寒热错杂证(12.07%),阴虚肠燥证(9.91%),血瘀肠络证(8.62%)。结论:因子分析方法得出溃疡性结肠炎的基本证候分类及分布规律,为该病的中医辨证分型客观化提供了一种新的方法和思路。

Objective： To explore the pattern of combination of symptoms, syndrome classification and syn- drome distribution by Factor analysis, and to provide the evidence for TCM syndrome differentiation in ulcera- tive colitis. Methods： Using the principles and methods of clinical epidemiology, factor analysis was applied to analyze the symptoms of 232 cases of ulcerative colitis. Results： The results of factor analysis showed that the basic TCM Syndromes of ulcerative colitis were seven types, the weakness of the spleen and stomach syndrome, large intestine damp heat syndrome, liver stagnation and spleen deficiency syndrome, X/in deficiency and in- testinal dryness syndrome, spleen kidney Yang deficiency syndrome, blood stasis syndrome and cold-heat com- plicated syndrome. The distribution of syndrome types was obtained as following： large intestine damp heat syn- drome （22.84%）, weakness of the spleen and stomach syndrome （17.24%）, spleen kidney Yang deficiency syn- drome （15.95%）, liver stagnation and spleen deficiency syndrome （13.36%）, cold-heat complicated syndrome （12.07%）, yin deficiency and intestinal dryness syndrome （9.91%） and blood stasis syndrome （8.62%）. Conclusion： The basic syndrome classification and distribution law of ulcerative colitis were derived from the factor analysis method, which provided a new method and thought for the syndrome differentiation and typing of the disease.
