摘要
目的探讨肩袖损伤MRI关节造影、MRI常规检查、超声的诊断准确度比较。方法选择新疆喀什地区第二人民医院经关节镜诊断为肩袖损伤入院治疗的30例患者,其中男性18例,女性12例;年龄36~68岁,平均年龄52.8岁。其中肩袖全层撕裂10例,部分撕裂17例,肩袖变性3例。病程2~8年。行MRI关节造影、MRI常规检查及超声检查,并根据检查方式的不同分为MRI造影组、MRI组、超声组。以关节镜检查结果为标准,比较3种检查的肩袖损伤检出情况。结果 MRI造影组诊断准确度为96.7%,MRI组为83.3%,超声组为86.7%;MRI造影组诊断准确度高于其他两组(P<0.05)。超声组与MRI组诊断准确度差异无统计学意义(P>0.05)。结论 MRI关节造影、MRI常规检查及超声对肩袖损伤均有良好的诊断效果,其中MRI关节造影诊断准确度最高。
Objective To explore the imaging diagnostic accuracy of rotator cuff injury in MRI arthroscopy, MRI routine examination and ultrasound. Methods A total of 30 patients with rotator cuff injury diagnosed by arthroscopy were enrolled,which included 18 males and 12 females, aged 36-68 years old with mean age of 52.8 years old. The arthroscopic rotator cuff with full-thickness tears was 10 cases, partial tears of 17 and 3 of rotator cuff degeneration, with the course was 2-8years. All of them were divided into MRI angiography group, MRI group and ultrasound group by different examination methods. The results of arthroscopic examination were used to compare 3 kinds of rotator cuff injury. Results The diagnostic accuracy were 96.7 %, 83.3 % and 86.7 % in MRI arthroscopy group, MRI group and ultrasound, respectively. The diagnostic accuracy in MRI arthroscopy was higher than that in the other 2 groups(P 〈 0.05), ultrasound and MRI group disease detection rate was no significant difference(P 〉 0.05). Conclusion It is demonstrated that MRI arthroscopy, MRI routine examination and ultrasound show the good effect in the diagnosis of rotator cuff injury, in which MRI arthroscopy with the highest diagnostic accuracy.
