中药联合氯氮平、舒必利治疗难治性精神分裂症的效果观察

Observing the efficacy of traditional Chinese medicine combined with clozapine and sulpiride in treatment of refractory schizophrenia

目的观察中药龙胆泻肝汤联合氯氮平、舒必利治疗难治性精神分裂症的临床效果。方法将医院收治的60例难治性精神分裂症患者,随机分为观察组和对照组,每组30例。对照组仅给予氯氮平治疗,观察组给予中药龙胆泻肝汤联合氯氮平、舒必利治疗。采用阳性与阴性症状量表(PANSS)、副反应量表(TESS)分别在治疗前,治疗后2周、4周、10周由主治医师进行评定,根据PANSS总减分率判定临床疗效,并观察2组治疗期间不良反应发生情况。结果观察组起效时间为(11.2±5.2)d,短于较对照组的(16.3±7.8)d(P<0.05)。观察组总有效率为90.0%,高于对照组的70.0%(P<0.05);治疗2周、4周后,观察组PANSS评分明显低于对照组(P<0.01);观察组不良反应发生率为10.0%,低于对照组的40.0%(P<0.05)。结论中药龙胆泻肝汤联合氯氮平、舒必利治疗难治性精神分裂症效果显著,起效时间快、不良反应轻。

Objective To observe the clinical effect of traditional Chinese medicine Longdan Xiegan decoction combined with clozapine and sulpiride in the treatment of refractory schizophrenia. Methods 60 patients with refractory schizophrenia were randomly divided into observation group and control group with each group of 30 cases. The control group received only clozapine treatment and the observation group were given traditional Chinese medicine,Longdan Xiegan decoction,combined with clozapine,sulpiride in the treatment. The positive and negative symptoms scale（ PANSS） and treatment Emergent Symptom Scale（ TESS） were evaluated by the attending physician before and 2 weeks,4 weeks and 10 weeks after treatment.The clinical curative effect was determined according to the total score reduction of PANSS. The incidence of adverse reactions during the treatment group was observed. Results The time interval before the medicine works for the observation group was（ 11. 2 ± 5. 2） d,which was shorter than that of the control group（ 16. 3 ± 7. 8） d（ P〈0. 05）. The total effective rate of the observation group was 90. 0%,which was higher than that of the control group（ P〈0. 05）. After 2 weeks and 4 weeks,the PANSS score of the observation group was significantly lower than that of the control group（ P〈0. 01）. The adverse reaction rate was 10. 0%,which was lower than that of the control group 40. 0%（ P〈0. 05）. Conclusion Traditional Chinese medicine Longdan Xiegan decoction combined with clozapine and sulpiride in the treatment of refractory schizophrenia is effective,the time interval before the medicine works is relatively short and the adverse reaction is relatively light.
