调神针刺法对原发性失眠患者睡眠质量及过度觉醒的影响

Effect of Tiaoshen Acupuncture Therapy on Sleep Quality and Hyperarousal State in Primary Insomnia

目的:研究针刺对原发性失眠(PI)患者睡眠质量及过度觉醒状态的影响。方法:60例PI患者,按1∶1比例随机分为治疗组(n=30)和对照组(n=30),治疗组给予调神法针刺百会、四神聪、本神、神庭、内关、神门,对照组予针刺无效穴臂臑、外关、风市等,两组均隔日针刺1次,疗程4周。采用匹兹堡睡眠质量指数(PSQI)和睡眠日志评估患者睡眠情况,采用过度觉醒量表(HAS)评估患者日间过度觉醒程度。结果:治疗组PSQI、睡眠日志的入睡潜伏期及入睡后觉醒次数、总睡眠时间、HAS的改善优于对照组(P<0.05)。结论:调神针刺法较针刺无效穴可有效降低原发性失眠患者过度觉醒状态,提高睡眠质量。

Objective： To observe the clinical effect of Tiaoshen acupuncture therapy on sleep quality and hyperarousal state in primary insomnia. Methods： Totally 60 patients with primary insomnia were recruited and randomized in a ratio of 1∶ 1 to the intervention group（ real acupuncture） and the control group（ sham acupuncture）. Du20,DU24,GB13,EXHN1,SP6,PC6,HT7 were punctured. Needle manipulation was applied to achieve ＂De Qi＂. Sham acupuncture was conducted by needling the acupoints of LI-14,LI-10,SJ4,SJ5,GB31,ST32 and GB31. ＂De Qi＂was avoided. Needle retention was 30 minutes. All the participants received acupuncture therapy with three times a week for 4 weeks. The Pittsburgh Sleep Quality Index（ PSQI） and sleep diaries（ including Sleep Latency-SL,Wake after Sleep Onset-WASO,total sleep time-TST） were used to assess sleep quality and Hyperarousal Scale（ HAS） was used to assess hyperarousal state. Results： Compared to those in the control group,PSQI and HAS scores,sleep latency,number of wake after sleep onset were decreased and the total sleep time was increased in the intervention group（ P〈0. 05）. Conclusion： The Tiaoshen acupuncture therapy is effective for decreasing the hyperarousal in patients with primary insomnia and is helpful for improving the sleep quality.
