窒息对新生儿细胞免疫功能的影响及机制

摘要 目的探讨新生儿窒息时细胞免疫功能(T 细胞亚群)的变化以及β-EP 是否参与了其中的免疫调节机制。方法分成观察组34例和对照组14例,均于生后第1d 和第7d 两次采血,分别检测β-EP、CD_3、CD_4及 CD_8等各项指标。结果与对照组相比,观察组新生儿细胞免疫功能明显抑制,且与窒息程度成正比,而β-EP 则显著升高;对照组为(211.46±82.96)ng/L,轻度窒息组为(346.24±75.80)ng/L,重度窒息组为(892.43±112.39)ng/L(P 均<0.01);两组第2次检测显示β-EP、CD_3、CD_4/CD_8等项差异有显著性意义(P<0.05);除 CD_8外,观察组β-EP 与各免疫指标检测差值均有显著相关性(P<0.05),相关系数(r)在0.463~0.719之间。结论窒息新生儿细胞免疫功能明显抑制,且与窒息程度成正比,β-EP 参与了其细胞免疫的抑制性调节。 To probe into the dynamic changes of cellular immune function(T-cell subpopulation)and to exploreβ-endorphin(β-EP)regulation mechanism in cellular immune.Methods All cases were divided into obser- vation group(34casas)and control group(14 cases).Two blood samples were collected at the first day and the seventh day after birth,,and β-EP,CD_3,CD_4 and CD_8 were detected.Results Compared with control group,cellular immune func- tion of observation group were inhibited obviously and which degree has direct ratio to asphyxia degree.The level of β-EP in- creased significantly(211.46±82.96)ng/L in control group,and(346.24±75.80)ng/L in mild asphyxia group, (892.43±112.39)ng/L in severn asphyxia group(P<0.01).There were significant difference in level of β-EP,CD_3, CD_4,CD_8 among groups(P<0.05).In observation group,them was significant correlation between the level of β-EP and immune indexes except CD_8,the correlation coefficient was 0.463-0.719.Conclusion Cellular immune function of as- phyxia infants were inhibited obviously and inhibited degree has direct ratio to asphyxia degree,β-EP played a role in inhib- ited regulation of the cellular immune function.
