从医院门诊药物咨询现状看咨询药师的培养

Comment on the training of counseling pharmacists based on current situations of medication consultation in our hospital

目的:通过分析门诊药物咨询室用药咨询记录,寻找咨询药师培养的方向和培养方法,促进门诊药物咨询室工作的快速发展。方法:回顾性整理某院2013年1月-2015年12月每周四单日的咨询记录表共1794份,对咨询问题的疑难程度、咨询内容、咨询患者的就诊科室来源、咨询涉及的药物个数及药物类型进行统计分析。结果:患者咨询复杂问题的数量及每次咨询的药品种类数均逐年增加。专业性比较强的咨询问题量虽逐年略有增加,但咨询内容还是以药品的供应(29.82%)及用法用量问题(22.85%)为主。总体上以内科就诊患者咨询问题为主,每年前三名均是心脏内科、消化科和内分泌科就诊过的患者。每年的咨询中涉及的药物种类都在60种以上,以中药居多,其次是降压药。结论:在建立咨询药师的岗位职责和考核标准的同时,更要做好咨询药师人才的培养,医院药学部门可设立咨询药师准入门槛,通过多种途径加强对咨询药师综合素质的培养,提高药学服务质量。

OBJECTIVE Records in the outpatient medication consultation room were analyzed,to find the direction and method of training counseling pharmacists,and to promote the rapid development of the outpatient medication consultation.METHODS A retrospective review of 1794 medication counseling records of Thursday weekly from January of 2013 to December of 2015 were performed,the difficult degree of advisory,consulting content,department sources of patients,numbers and types of drug were statistically analyzed.RESULTS The proportion of complex problems and the number of drug of each consultation were increasing year by year.Although the amount of professional consulting issues increased slightly,the advisory was dominated by drug supply（29.82%）and the administration（22.85%）.Overall patients were internal medicine problems,the top three were department of cardiology,gastroenterology and endocrinology every year.The annual consultation types of drugs involved were more than 60 species,the majority was traditional Chinese medicine,followed by antihypertensive drugs.CONCLUSION When the job responsibilities and assessment criteria of counseling pharmacists are established,training of counseling pharmacists should be strengthened.Department of pharmacy may establish a threshold of counseling pharmacists.Comprehensive quality of counseling pharmacist can be cultured by a variety of ways.
