血清CD64、CD54、sTREM-1及炎性因子在儿童细菌性肺炎进展中的表达水平分析 被引量:13

摘要 目的探究血清CD64、CD54、sTREM-1及炎性因子在儿童细菌性肺炎进展中的表达情况分析。方法选取医院2012年1月-2015年12月收治的细菌性肺炎患儿114例设为观察组,同期医院接受健康检查的儿童120名设为对照组,收集观察组患儿痰液行痰培养及药敏试验,抽取两组儿童静脉血,检测CD64、CD54、sTREM-1、CRP、TNF-α及IL-6水平。结果 114例细菌性肺炎的患者痰培养共检出病原菌132株,其中革兰阳性菌47株,革兰阴性菌85株;观察组血清中CD64、CD54、sTREM-1、CRP、TNF-α及IL-6均明显高于对照组水平,差异有统计学意义(P<0.05)。结论血清中CD64、CD54、sTREM-1及炎性因子含量与儿童发生细菌性肺炎有密切的相关性,可以作为预测疾病发生发展的指标,并且应当在第一时间行痰液培养,发现病原菌,精准选用合适的抗菌药物,提高早期治疗效果。 OBJECTIVE To explore the expression of serum CD64,CD54,sTREM-1and inflammatory cytokines in childrne with bacteiral pneumonia.METHODS Totally 114 children with bacteiral pneumonia who were treaed in hospitals from Jan 2012 to Dec 2015 were chosen as the observation group,meanwhile,120 healthy children who received physical examination were set as the control group.The sputum culture was carried out for the children in the observation group,the drug susceptibility testing was performed,and the venous blood specimens were drawn from the two groups of children so as to detect the levels of CD64,CD54,sTREM-1,CRP,TNF-α,and IL-6.RESULTS A total of 132 strains of pathogens were isolated from the sputum specimens of the 114 children with bacterial pneumonia,including 47 strains of gram-positive bacteria and 85 strains of gram-negative bacteria.The levels of serum CD64,CD54,sTREM-1,CRP,TNF-α,and IL-6of the observation group were significantly higher than those of the control group(P〈0.05).CONCLUSIONThe incidence of bacterial pneumonia in the children is closely associated with the levels of serum CD64,CD54,sTREM-1,and inflammatory cytokines,which can be used as the indicators for prediction of development and progression of the disease,and it is necessary to carry out the sputum culture in a timely manner and identify the pathogens so as to reasonably use antibiotics and improve the early therapeutic effect.
