膝骨关节炎患者静态平衡能力的影响因素分析 被引量:10

摘要 目的探讨膝骨关节炎(KOA)患者病程、体质指数(BMI)、疼痛程度、骨关节炎指数(WOMAC)量表积分与静态平衡能力的相关性。方法选取2016年1—10月社区招募和福建中医药大学附属第三人民医院门诊就诊的KOA患者50例为研究对象。采用疼痛视觉模拟评分(VAS)评价膝关节疼痛程度,采用WOMAC量表评价膝关节功能,采用Pro-Kin 254P型平衡测试系统评价睁眼、闭眼静态平衡能力。结果 KOA患者病程与睁眼静态平衡指标前后方向标准偏差、横向标准偏差、前后方向平均运动速度、横向平均运动速度、运动椭圆面积、运动长度呈正相关(P<0.05);BMI与闭眼静态平衡指标横向标准偏差、运动椭圆面积呈正相关(P<0.05);VAS与睁眼静态平衡指标y轴平均压力中心呈正相关,与罗伯长度呈负相关(P<0.05);WOMAC量表日常功能活动积分与闭眼静态平衡指标y轴平均压力中心呈正相关(P<0.05)。结论 KOA患者病程、BMI、疼痛程度、日常功能活动能力与静态平衡指标有关,患者静态平衡能力对视觉的依赖较大。 Objective To investigate the correlation of course of knee osteoarthritis( KOA),body mass index( BMI),degree of pain,scores of Western Ontario and McMaster Universities Osteoarthritis Index( WOMAC) with the static balance ability in patients with KOA. Methods From January to October 2016,50 participants were recruited for this study,including KOA community residents and KOA outpatients of the Third People' s Hospital Affiliated to Fujian University of Traditional Chinese Medicine. The status of knee pain was assessed by Visual Analogue Scale( VAS),knee joint function was assessed by WOMAC,and static balance ability in both eyes-open and eyes-closed conditions was assessed by Pro-Kin 254P system. Results The course of KOA was found to be positively correlated with the values of balance parameters of SD of ap,SD of la,AS of ap,AS of la,Area,Length in eyes-open condition( P〈0. 05). BMI was positively associated with the values of static balance parameters of the SD of la,Area under eyes-closed condition( P〈0. 05). VAS scores were positively correlated with the static balance parameter of C. O. P( y)( P〈0. 05),but negatively correlated with Romberg length( P〈0. 05) under eyes-open condition. There was a positive correlation between static balance parameter of C. O. P( y) and WOMAC function subscale scores under eyes-closed condition( P〈0. 05). Conclusion The poor static balance ability in KOA patients is associated with prolonged course of KOA,increased BMI and pain,reduced function ability,and the patient' s static balance ability has a greater reliance on vision.
