加强孕期规范管理对高龄孕妇妊娠结局的影响

Influence of strengthening gestational standardized management on pregnancy outcome in advanced maternal age

目的探讨加强孕期规范管理对高龄孕妇妊娠结局的影响。方法选择2014年2月~2016年8月在我院产检的高龄孕妇190例为观察组,实施孕期规范管理;选择同期在我院门诊就诊的190例高龄产妇为对照组,未接受孕期规范管理。比较两组孕期合并症、并发症和妊娠结局。结果观察组妊娠期糖尿病、妊娠期高血压、妊娠期贫血、产后出血、胎膜早破、产褥期感染等发生率均低于对照组,差异有统计学意义(P<0.05);两组前置胎盘发生率比较,差异无统计学意义(P>0.05);观察组中低体重儿、早产、巨大儿、出生缺陷、新生儿窒息、胎儿畸形、围生儿死亡等发生率与对照组比较明显降低,差异有统计学意义(P<0.05)。结论对高龄孕妇加强孕期规范管理,能有效降低母婴不良结局发生,值得临床推广。

Objective To investigate the influence of strengthening gestational standardized management on pregnancy outcome in advanced maternal age （AMA）.Methods 190 AMA patients received antenatal examination in our hospital from February 2014 to August 2016 were selected as observation group,and they were given gestational standardized management;and another 190 AMA outpatients without gestational standardized management in our hospital were selected as control group.The accompany diseases during pregnancy,complications and pregnancy outcomes of two groups were compared.Results The incidence of gestational diabetes mellitus,gestational hypertension,anemia during pregnancy, postpartum hemorrhage,premature rupture of membrane,puerperal infection of observation group was lower than that of the control group （P〈0.05）;there was no statistical difference in the incidence of placenta previa between two groups （P〉 0.05）;the incidence of low birth weight infant,premature birth,macrosomia,birth defect,neonatal asphyxia,fetal malformation,perinatal mortality of observation group significantly decreased compared with control group （P〈0.05）.Conclusion Strengthening gestational standardized management for AMA can effectively decrease adverse pregnancy outcomes,which is worthy of clinical promotion.
