小剂量氯沙坦联合固肾降酸方治疗早期原发性痛风性肾病的临床研究

The clinical study of low dosage of Losartan combined with the Prescription of Strengthening Kidney and Reducing Acid treating the early primary gouty nephropathy

目的探讨小剂量氯沙坦联合固肾降酸方治疗早期原发性痛风性肾病的临床效果。方法选取2014年9月~2016年9月于我院住院诊治的100例早期原发性痛风性肾病患者,依据随机对照分组原则将其分为对照组与观察组,每组50例,在相同的基础治疗的基础上,对照组患者予以别嘌醇治疗,观察组患者则予以小剂量氯沙坦联合固肾降酸方治疗,连续治疗3个月后,比较两组患者的临床症状改善情况、临床指标改善情况及临床疗效等。结果治疗后,观察组患者的临床症状、肾功能指标改善程度均明显优于对照组,差异有统计学意义(P<0.05);治疗后,观察组患者的治疗总有效率显著高于对照组,差异有统计学意义(P<0.05)。结论小剂量氯沙坦联合固肾降酸方治疗早期原发性痛风性肾病可以有效改善患者的临床症状、肾功能、尿酸、24 h尿蛋白定量等近期疗效指标,保护肾脏功能,提高临床治疗总有效率,是一种可行的治疗手段,值得临床上进一步应用推广。

Objective To investigate the clinical effect of low dosage of Losartan combined with the Prescription of Strengthening Kidney and Reducing Acid treating the early uric acid nephropathy.nethods A total of 100 patients with early uric acid nephropathy admitted into our hospital from September 2014 to September 2016 were selected and divided into two groups：control group and observation group with 50 cases in each group according to the principle of randomized controlled classification.Based on the basis of the same treatment,the patients of the control group was treated with Allopurinol and the patients of the observation group was treated with low dosage of Losartan combined with the prescription of Strengthening Kidney and Reducing Acid.After 3 months of continuous treatment,the clinical symptoms improvement,the improvement of clinical indexes and the clinical efficacy between the two groups were com- pared.Results After treatmrent,the improvement of clinical symptoms,renal function index and 24 h urine protein quan- titative were significantly better than those of the control group after treatment and the difference was statistically sig- nificant （P〈0.05）.After treatment,the total effective rate of the observation group was significantly higher than that of the control group and the difference was statistically significant （P〈O.OS）.Conclusion Low dosage of Losartan combined with the Prescription of Strengthening Kidney and Reducing Acid treating the early uric acid nephropathy can clinical symptoms, renal function,uric acid,24 h urinary protein and other short-effect index,and it can protect renal function,improve the total efficiency rate of clinical treatment,and it is a feasible treatment method.It is worthy of further clinical application and promotion in clinic.
