补肾暖冲汤联合针灸治疗脾肾两虚型卵巢早衰的临床研究

Study on Bushen-Nuanchong decoction combined with acupuncture and moxibustion on premature ovarian failure with deficiency of spleen and kidney

目的观察补肾暖冲汤联合针灸治疗脾肾两虚型卵巢早衰的临床疗效及对性激素水平的影响。方法将56例脾肾两虚型卵巢早衰患者随机均分为对照组与观察组,对照组给予克龄蒙口服,观察组给予自拟补肾暖冲汤联合针灸治疗,2组均治疗3个月,停药后随访3个月。治疗结束后评估2组临床疗效,观察2组治疗前后临床症状Kupperman评分变化情况,检测2组治疗前及停药后3个月血清卵泡刺激素(FSH)、雌二醇(E_2)及黄体生成素(LH)水平,同时分析临床疗效、Kupperman评分与性激素水平的相关性。结果观察组治愈率、总有效率均明显高于对照组(P均<0.05);治疗后2组Kupperman评分均明显降低(P均<0.05),且观察组Kupperman评分明显低于对照组(P<0.05);停药后3个月,2组FSH、LH水平均明显降低(P均<0.05),E_2水平均明显升高(P均<0.05),且观察组各指标改善情况均优于对照组(P均<0.05)。临床疗效与FSH、LH水平呈明显正相关(0<r<1,P均<0.05),与E_2水平呈明显负相关(-1<r<0,P<0.05);临床症状Kupperman评分与FSH、LH水平呈明显正相关(0<r<1,P均<0.05),与E_2水平呈明显负相关(-1<r<0,P<0.05)。结论补肾暖冲汤联合针灸治疗脾肾两虚型卵巢早衰疗效确切,可明显调节性激素水平,值得推广应用。

Abstract： Objective It is to observe the clinical effect of Bushen-Nuanchong decoction combined with acupuncture and moxibustion on spleen and kidney double deficiency of premature ovarian failure（POF） and influence on the secretion of hormone level of the patients. Methods 56 patients with POF with spleen and kidney deficiency were randomly divided into control group and observation group, the control group were treated by conservative western medicine, the observation group was treated with Bushen-Nuanchong decoction combined with acupuncture ＋ moxibustion, both groups were treated for 3 months, and were followed up for 3 months after drug withdrawal. The clinical curative effects were evaluated in both groups at the end of treatment, the changes of Kupperman scores of clinical symptoms before and after treatment were observed, and the levels of the serum FSH, E2 and LH were detected before treatment and in 3 months after withdrawal, the correlation of curative effect with Kupperman scores and sex hormone were analyzed at the same time. Results The clinical cure rate and total effective rate were significantly higher in observation group than that of the control group （P 〈 O. 05）. After treatment, Kupperman scores were decreased in both groups （P 〈 0.05） , and the scores in observation group were lower than that in control group （ P 〈 0. 05 ）. In 3 months after withdrawal, the levels of FSH, LH were decreased while that of E2 was increased in both groups （P 〈 0. 05）, and the improvements of these indexes in observation group were better than that in control group （ P 〈 0.05 ）. The curative effect was significantly positively related with the levels of FSH, LH （ 0 〈 r 〈 1, P 〈 0.05 ） , and negatively related with that of E2 （ - 1 〈 r 〈 0, P 〈 0.05） ; the Kupperman scores of clinical symptoms was significantly positively related with the levels of FSH, LH （0〈r〈1, P〈0.05）, and negatively related with that of E2（ -1 〈r〈0, P〈0.05）. Conclusion Bushen-Nuanchong decoction combined with acupuncture and moxibustion which could regulate sex hormone had good curative effect on POF with spleen and kidney double deficiency, and it was worthy of popularization and application in clinic.
