导尿管与COOK宫颈扩张球用于晚期妊娠促宫颈成熟并引产的临床研究

Clinical study of catheter and COOK cervical dilating ball in cervical ripening and induction of labor in late pregnancy

目的探讨导尿管与COOK宫颈扩张球用于晚期妊娠促宫颈成熟并引产的临床效果。方法 2015年1月~2016年12月68例晚期妊娠引产产妇,随机分两组,导尿管组采用导尿管进行引产治疗,宫颈扩张球组采用COOK宫颈扩张球进行引产,比较两组患者宫颈成熟度、剖宫产率、引产出血量、引产时间、引产效果、住院时间和治疗费用。结果 (1)宫颈扩张球组宫颈成熟度为(6.88±1.38)分,明显高于治疗前,导尿管组宫颈成熟度为(6.90±1.38)分,明显高于治疗前,治疗前后比较差异有统计学意义(P<0.05)。两组患者治疗后的宫颈成熟度和剖宫产率比较差异无统计学意义(P>0.05)。(2)两组患者的宫颈裂伤和引产成功率、引产出血量、住院时间比较差异无统计学意义(P>0.05)。宫颈扩张球组治疗费用明显高于导尿管组,数据比较差异有统计学意义(P<0.05)。结论导尿管与COOK宫颈扩张球用于晚期妊娠促宫颈成熟并引产的临床效果显著,并且二者效果相当,但是导尿管比COOK宫颈扩张球治疗成本低,有利于降低患者的经济压力,更实用。

Objective To investigate the clinical effect of catheter and COOK cervical dilating ball for cervical ripening and induction of labor in late pregnancy. Methods 68 cases of maternal labor in late pregnancy from January 2015 to December 2016 were randomly divided into two groups.Patients in catheter group were treated with urethral catheter to induce labor, and patients in cervical dilation ball group were treated with COOK cervical dilation ball induced labor.The cervical maturity,cesarean section rate,abortion bleeding,induction time,induction effect the hospitalization time,and treatment costs of the two groups we~＇e compared. Results （1）The cervical maturity of cervical dilation ball group was （6.88±1.38）,which was significantly, higher than that before treatment.The cervical maturity of catheter group was （6.90±1.38）,which was significantly higher than that before treatment,there had statistical significance（P 〈 0.05）.There was no significant difference in cervical maturity and cesarean section rate after treatment between the two groups（P 〉0.05）.（2）There was no significant difference between the two groups in the rate of cervical laceration,the rate of induced labor, the amount of induced labor and the length of stay in hospital（P 〉 0.05）.The cost of cervical dilating ball group was significantly higher than that of catheter group,and the difference between the two groups was statistically significant （P 〈 0.05）. Conclusion The clinical effects of catheter and COOK cervical dilating ball in cervical ripening and induction of labor in late pregnancy are significant.And the two are equally effective.But the cost of catheter is less than the COOK cervical dilating ball.It helps to reduce the patient＇ s financial pressure,more practical.
