抗NMDA受体脑炎误诊为病毒性脑炎原因分析 被引量:5

摘要 目的分析抗N-甲基-D-天冬氨酸(N-methyl-D-asparate receptor,NMDA)受体脑炎的临床特点和误诊原因。方法回顾性分析我科收治的1例误诊为病毒性脑炎的抗NMDA受体脑炎病例资料。结果本例17岁女性,6个月前因发作性肢体抽搐就诊外院,诊断癫痫大发作,予对症处理症状未能控制转我院神经内科,诊断为病毒性脑炎,予联合抗癫痫治疗,症状控制。4 h前症状再发第2次入我院神经内科,行头颅MRI系列检查及妇科彩超均未见异常,视频脑电图示广泛慢波增多,仍按病毒性脑炎予联合抗癫痫治疗控制癫痫发作,效果不佳。病程中出现精神症状,予对症治疗效果不佳。完善脑脊液及血清自身免疫性脑炎相关抗体检测,脑脊液抗NMDA受体抗体(++),血清抗NMDA受体抗体(+),确诊为抗NMDA受体脑炎。予人免疫球蛋白、糖皮质激素、环磷酰胺等治疗,症状好转出院。结论抗NMDA受体脑炎临床少见且表现多无特异性,易误诊为病毒性脑炎,尽早行血清和脑脊液自身免疫性脑炎相关抗体检测有助于确诊。
