摘要
从经济学角度分析,医师选择多点执业政策遇冷的原因主要是闲暇程度、工资率、流动成本和职业性人力资本投资偏好等因素。若不考虑医师在多点执业过程中的流动成本,医师是否选择多点执业取决于医师在主职单位的最低劳动时间和最优劳动时间的关系;若医师主职单位规定的最低劳动时间大于医师最优劳动时间,则只有兼职单位的工资水平达到较高水平医师才愿意接受多点执业;若医师因多点执业而产生了流动成本,则流动成本会降低医师多点执业的可能性。由此需加大医师执业制度改革、推动落实分级诊疗制度和提升医师诊疗收入等政策的有效推进。
Using economics, this paper analyses the factors affecting physicians'dual practice. We find that the main factors include degree of leisure, wage rates, turnover cost and human capital investment preference in occupational work. This paper proposes three viewpoints. Firstly, without turnover cost,physicians decide whether to take dual practice by the relationship between the minimum working hours of the main job and the optimal working hours. Secondly, if the minimum working hours required by the main job are greater than the optimal working hours, physicians would accept more practice only when the wages level of dual practice reach a certain level. Thirdly, turnover costs reduce the likelihood and the benefits of dual practice, and increase the likelihood of reducing working hours. This paper puts forward the countermeasures to build new relationship between the hospital and physicians and to increase the diagnoses income of physicians by establishing a hierarchical medical system.
