高血压心力衰竭病人住院期间自我感受负担现状及其影响因素

Status quo of self-perceived burden and influencing factors in hospitalized hypertension patients complicated with heart failure

[目的]调查高血压心力衰竭病人住院期间自我感受负担现状及其影响因素。[方法]采用自我感受负担量表(SPB)对我院收治的276例高血压合并心力衰竭住院病人进行调查。[结果]高血压心力衰竭病人住院期间SPB得分为(33.05±5.06)分,处于中度感受负担水平,SPB量表中"担心治疗需要很多钱"和"担心照顾人员太过劳累"得分最高。不同分类的医保情况、改良Barthel指数、心功能分级、高血压分级、有无合并症、家庭收入能否支撑医疗费用和照顾者身体状况的病人SPB得分存在差异(P<0.05)。多因素Logistic回归分析结果显示,家庭收入能否支撑医疗费用、医保情况、Barthel指数和照顾者身体状况4个因素是高血压心力衰竭病人住院期间自我感受负担的影响因素(P<0.05)。[结论]高血压心力衰竭病人住院期间自我感受负担明显,家庭收入、医保、Barthel指数和照顾者身体状况是病人住院期间产生负担感觉的主要因素。

Objective： To investigate the status quo of self - perceived burden and its influencing factors in hyper-tension patients complicated with heart failure during hospitalization. Methods： A Self - Percwas used to investigate 276 hypertension patients complicated with heart failure in our hospital. Results： The SPB score was（33. 05± 5. 06） in hospitalized hypertension patients with heart failure andwas at amoderate bu- den level. Therewere the highest score in “high costworrying treatment” and “worrying thatoo tired,,（P〈0. 05）. There was a significant difference in SPB score among patients wimedical insurance different Barthel indexes, the grades of heart function, the classifications of hpresence or absence of complications, whether the family income could support the medical expenses or not and the physical condition of the caregivers. Multivariate logistic regression analysis showed that family factors could support medical expenses ,health insurance , Barthel index and physical condition of caregivers were the in-fluencing factors of self - perceived burden during hospitalization of hypertension patients with heart failure（P〈0. 05）. Conclusions ： Self - perception of hypertension and heart failure patients during hospitalization was obvi-ous. Family income ？ health insurance , Barthel index and caregiver physical condition were the main reasons for the burden of patients during hospitalization.
