摘要
目的探讨脑胶质瘤相关同原癌基因1(Gli1)在幽门螺杆菌(H.pylori)相关胃癌及癌前病变中的表达及意义。方法采用改良Giemsa染色法对160例胃癌及癌前病变组织进行H.pylori检测并分组;免疫组织化学二步法检测各组标本中Gli1蛋白的表达;分析H.pylori感染与Gli1蛋白表达的关系。结果 Gli1蛋白在胃癌及癌前病变各组间表达有显著差异(H=39.85,P<0.001),胃癌组显著高于慢性浅表性胃炎及化生性萎缩性胃炎组(P均<0.001),胃癌组与异型增生组比较差异无统计学意义(P>0.05);各病变组再分为H.pylori阴性及阳性亚组,在化生性萎缩性胃炎组中,H.pylori阳性亚组Gli1的表达显著高于H.pylori阴性亚组,其余病变组中,H.pylori阴性及阳性组间无显著差异(P>0.05)。结论 Gli1蛋白在胃癌组织中异常高表达;H.pylori感染可能在胃黏膜癌前病变阶段上调Gli1蛋白的表达,以促进胃黏膜癌变的发生。
Objective To investigate the expression of Gli1 in H.pylori-related gastric cancer and pre-cancerous lesions. Methods A total of 160 tissue specimens of precancerous lesions and gastric cancer were divided into H.pylori positive and negative groups by modified Giemsa staining. Expression of Gli1 was as-sayed by immunohistochemistry. Furthermore, the relationship between Gli1 protein expression and H.pylori-infection was analyzed. Results The expression of Gli1 was significantly different among gastric lesions (H=39.85, P〈 0.001), and gastric cancer group was significantly higher than chronic superficial gastritis and metaphastic atrophy gastritis group(P〈0.001), whereas the difference was not significant when compared with dysplasia group (P〉0.05). In the metaphastic atrophy gastritis group, the expression of Gli1 in the H.pylori positive subgroup was significantly higher than the H.pylori negative subgroup. There was no significant dif-ferences between all other H.pylori negative and positive subgroups (P〉0.05). Conclusion Gli1 was highly expressed in gastric cancer. In the precancerous stage, H.pylori infection may increase the expression of Gli1 protein in the gastric mucosa, which may promote gastric carcinogenesis.
